Studien weisen darauf hin, dass Frauen mit einem PCOS häufiger einen niedrigeren Vitamin D Status haben. Ein niedrigerer Vitamin D Status von Frauen während der Schwangerschaft wird mit dem gehäuften Auftreten von perinatalen Komplikationen in Verbindung gebracht. Das Ziel der Arbeit ist es, den Vitamin D Status bei Frauen mit und ohne PCOS zum Zeitpunkt der Geburt und bei den Neugeborenen dieser Frauen zu erheben. Im zweiten Schritt werden die Vitamin D Spiegel mit dem Schwangerschaftsoutcome assoziiert.\r\n\r\nMethoden\r\nEs handelt sich um eine prospektive Studie, die an der Universitätsklinik für Frauenheilkunde und Geburtshilfe an der Medizinischen Universität Graz durchgeführt wurde. Es wurde der Vitamin D Status erhoben und ein möglicher Zusammenhang der mütterlichen und kindlichen Vitamin D Werte mit folgenden Schwangerschaftskomplikationen untersucht: Komplikationen der Mutter/Kind, Geburtsmodus, vorzeitige Wehen, präexistente Hypertonie/Schwangerschafts induzierte Hypertonie/Präeklampsie, Gestations Diabetes Mellitus (GDM), früher vorzeitiger Blasensprung (PPROM) , Fruchtwassermenge, Versorgung, Intrauteriner Fruchttod (IUFT), Intrauterine Wachstumsretardierung (IUGR), Frühgeburt, Azidose, low for gestational age (LGA) und small for gestational age (SGA), und Auftreten einer Laktoseintoleranz. \r\n\r\nErgebnisse\r\nDie mütterlichen Vitamin D Spiegeln haben sich nicht signifikant zwischen Frauen mit PCOS und ohne PCOS unterschieden (p=0,998). Auch die Vitamin D Spiegel der Kinder zeigten sich nicht signifikant (p=0,692). Das Auftreten einer Vitamin D Defizienz (<20 ng/ ml) war in beiden untersuchten Gruppen hoch. Die Korrelation zwischen den mütterlichen und kindlichen Vitamin D Spiegeln zeigte eine stark positive Korrelation von 0,84 bei Frauen mit PCOS und 0,83 ohne PCOS. Die neonatalen Vitamin D Werte in der Nabelschnur betrugen 77% der Serum Vitamin D Spiegel der Mütter. Im Vergleich mit der gesunden Kohorte zeigte sich das Risiko für mütterliche Komplikationen bei den PCOS Frauen erhöht (48% versus 65%; p=0,009), während sich bei den neonatalen Komplikationen kein signifikanter Unterschied darstellte (22% versus 22%; p=1,0). Die Vitamin D Spiegel waren jedoch bei Müttern und Säuglingen mit und ohne perinatale Komplikationen ähnlich. \r\n\r\nSchlussfolgerung\r\nObwohl der Anteil von Frauen und Säuglingen mit Vitamin-D Mangel bei Frauen mit PCOS und ohne PCOS hoch war, scheint es, dass das Auftreten von perinatalen Komplikationen nicht dadurch beeinflusst wird. Besonders häufig treten Epilepsieformen bei Minderjährigen auf, bei diesen Patienten besteht auch die besondere Gefahr von chronischen Hirnschäden und Entwicklungsverzögerungen. Um diesen Folgen entgegenzuwirken ist die angemessene Behandlung epileptischer Erkrankungen von größter Bedeutung.\r\n\r\nMethode:\r\nDas Ziel dieser Diplomarbeit ist es, einen Überblick über die Charakteristika der Epilepsie bei Minderjährigen herzustellen sowie Besonderheiten in der Behandlung solcher Fälle aufzuzeigen. Als Methode dafür wurde eine Literaturrecherche gewählt. Als Quellen dienten medizinische Fachbücher aus den Gebieten Pathophysiologie, Pharmakologie, Neurologie und Pädiatrie, aktuelle Leitlinien, Internetquellen sowie aktuelle klinische Studien und Reviews.\r\n\r\nErgebnisse:\r\nDie wichtigste Erkenntnis in der Epilepsiebehandlung ist die Feststellung, dass eine auf das Individuum angepasste Therapie nötig ist. Eine Vielzahl von Faktoren nimmt Einfluss auf die Therapiewahl, darunter fallen Alter und Geschlecht der Patienten, die vorliegende Epilepsieform sowie das individuelle Auftreten von Nebenwirkungen. Eine große Rolle in der Zukunft der juvenilen Epilepsiebehandlung kommt der fortlaufenden pharmakologischen Forschung zu. Als Folge dieser Forschungen werden neue Wirkstoffe entwickelt, die entweder bei speziellen Erkrankungsformen Wirkung zeigen oder weniger Nebenwirkungen aufweisen. Dadurch bietet sich die Möglichkeit, alte, mit schweren Nebenwirkungen verbundenen Therapieformen nicht mehr als First-Line-Therapie einsetzten zu müssen. Venöse Thromboembolien treten mit einer Inzidenz von 1:1000 auf und waren alleine im Jahr 2018 für über 4000 Todesfälle in Österreich verantwortlich. Sowohl als Schlaganfallprophylaxe als auch zur Prävention und Therapie thromboembolischer Ereignisse ist eine\r\nantikoagulatorische Therapie unerlässlich. Über 60 Jahre waren dabei die Vitamin- K-Antagonisten die einzig verfügbaren Arzneimittel. Mit Einführung der direkten oralen Antikoagulantien im Jahr 2008 eröffneten sich jedoch neue Therapieoptionen. Seitdem steigt die Anzahl der PatientInnen, die mit DOAKs behandelt werden, stetig.\r\n\r\nMethoden\r\nDiese Diplomarbeit stellt eine Übersichtsarbeit in Form einer systematischen Literaturrecherche dar. Dabei wurde hauptsächlich auf Fachliteratur aus Lehrbüchern, Fachzeitschriften, Internetdatenbanken (PubMed) sowie online verfügbare Informationen, zurückgegriffen.\r\n\r\nSchlussfolgerung\r\nKlare Vorteile der DOAKs sind fixe Dosierung, keine Therapieüberwachung, keine Nahrungsmittelinteraktionen, weniger Medikamenteninteraktionen, kein Bridging sowie ein rascher Wirkungsbeginn bei gleichzeitig raschem Wirkungsende. Durch die Entwicklung spezifischer Antidote konnte das Sicherheitsprofil im Umgang mit diesen Substanzen weiter verbessert werden. Auch die DOAK Zulassungsstudien bestätigen die Gleichwertigkeit bzw. Symptome dieser Krankheit, die bis zu 10% aller Frauen im gebärfähigen Alter betrifft, sind unter anderem chronischer Beckenbodenschmerz, Dysmenorrhoe, Dyspareunie oder Infertilität. Bei laparoskopischer Diagnose kann in der Peritonealflüssigkeit und im Gewebe der Endometrioseherde ein vermehrtes Vorkommen von Prostaglandinen nachgewiesen werden. Prostaglandine stellen eine große Gruppe von Gewebshormonen mit einem breiten Wirkspektrum dar.\r\nZiel dieser Literaturarbeit ist es, Antworten über die Bedeutung der Prostaglandine beim Krankheitsbild der Endometriose zu liefern. Die Zielsetzung erhält besondere Bedeutung, da die Kausalität und die Pathomechanismen dieser Erkrankung noch weitestgehend unbekannt sind.\r\nEine umfangreiche Recherche vorherrschender Literatur verdeutlicht eine Beteiligung der Prostaglandine an der Pathophysiologie der Endometriose. Unter allen Prostaglandinklassen kommt vor allem Prostaglandin E2 (PGE2) eine wichtige Funktion zu. PGE2 fördert die Pathogenese und Erhaltung der Krankheit durch Induktion der Steroidbiosynthese, Stimulation der Angiogenese, Förderung des Wachstums durch Steigerung der Zellproliferation und Immunsupression durch Hemmung der Phagozytose in Makrophagen.\r\nMedikamentöse Behandlungsmethoden der Endometriose zielen großteils auf die östrogensteigernde Wirkung der Prostaglandine ab. Durch Herbeiführen eines hypoöstrogenen Zustandes soll die Atrophie der Endometrioseläsionen erzielt werden. Die Verwendung eben jener Medikamente, wie z.B. GnRH-Agonisten, Androgene, kombinierte hormonelle Kontrazeptiva oder Aromatase-Inhibitoren, stellt jedoch lediglich eine symptomatische Therapie dar. Eine Sensibilisierung der Öffentlichkeit gegenüber Herz-Kreislauf-Stillständen und den damit einhergehenden Wiederbelebungsmaßnahmen durch Thoraxkompressionen oder der Verwendung eines Defibrillators, ist deshalb wichtig. Diese Arbeit soll einen Überblick über Erste Hilfe bieten, sowie über die Relevanz von kurzen Einheiten in lebensrettenden Sofortmaß-nahmen. Im Rahmen des Projektes ,,Drück mich“ wurden Teilnehmerinnen/Teilnehmer nach kurzen Einheiten zum Thema „Lebensrettende Sofortmaßnahmen“ befragt. Medizinstudierende versuchten in Einheiten von einer Dauer zwischen ca. 20-60 Minuten der Laiin/den Laien die Basismaßnahmen der Ersten Hilfe näherzubringen. Diese Ergebnisse wurden mittels Diagrammen veranschaulicht. Auf die Befragung hinsichtlich der Durchführung einer CPR gaben 74,24% der 132 Teilnehmerinnen/Teilnehmer an, diese eher durchzuführen. Die Anwendung eines Defibrillators würden sich 84,1% der 132 Teilnehmerinnen/Teilnehmer eher zutrauen. Somit traut sich der Großteil nach den Einheiten zu, mit einer kardiopulmonalen Reanimation im Anlassfall zu beginnen sowie einen Defibrillator zu verwenden.
