Um die Übertragung und damit die weitere Ausbreitung der Infektionskrankheit einzudämmen, \r\nwurde gemäß Artikel 7 und 8 des COVID-19 Gesetzes in Österreich ab dem 16.03.2020 ein \r\nlandesweiter Lockdown verhängt. \r\nTrotz des nachgewiesenen Einflusses der Pandemie auf die Essgewohnheiten von Kindern und \r\nJugendlichen ist derzeit keine Untersuchung des Effekts auf PatientInnen mit Anorexia nervosa (AN) \r\noder atypischer Anorexia nervosa (AAN) in Österreich bekannt. \r\nAus diesem Grund widmet sich die folgende Arbeit der Fragestellung, ob die Sars-CoV-2 Pandemie \r\nund die damit verbundenen Beschränkungen des täglichen Lebens eine Auswirkung auf die Inzidenz \r\nund den Behandlungserfolg von Anorexia nervosa hat. Mithilfe einer retrospektiven Studie am \r\nUniversitätsklinikum für Kinder- und Jugendheilkunde der Medizinischen Universität Graz wird die \r\nBeeinflussung von äußeren Gegebenheiten, wie das Pandemiegeschehen, persönliche \r\nEinschränkungen und Isolation auf die Entstehung und den Verlauf der Essstörung untersucht. \r\nÜberraschenderweise konnten keine signifikanten Unterschiede zwischen den PatientInnenkollektiven \r\nder Zeiträume vor und während der Pandemie festgestellt werden. Allerdings wurde während COVID19 eine Zunahme bei den Erstdiagnosen erhoben. Aus subjektiver Sicht nimmt die Anzahl der Schmerzpatienten stetig zu, während es andererseits aufgrund von Kosteneinsparungen zu vermehrten Ambulanzschließungen und Verringerungen im Behandlungsspektrum kommt. Dadurch müssen Patienten von Ambulanzen abgelehnt werden, länger auf einen Termin warten und zunehmend weitere Fahrtstrecken in Kauf nehmen, um eine Behandlung in Anspruch nehmen zu können. All diese Punkte lassen eine zunehmende medizinische Minderversorgung von SchmerzpatientInnen vermuten und sollen nun im Rahmen dieser Arbeit objektiviert belegt werden. \r\n\r\nMethoden\r\nIm Rahmen dieser Arbeit wurde eine Onlineumfrage durchgeführt und ausgewertet. \r\nDie Umfrage besteht aus 21 Fragen zum Betrieb der Schmerzambulanz mit der Bitte, diese von der jeweiligen Klinik zu beantworten. Diese Umfrage wurde online per E-Mail an die Leiter der 109 österreichischen Kliniken mit einer vorhandenen Schmerzambulanz verschickt und war im Zeitraum vom Mai 2022 bis Juni 2023 zur Bearbeitung freigegeben. \r\n\r\nErgebnisse\r\nZweiundneunzig der 109 (Rücklaufquote von 84%) befragten Kliniken nahmen an der Umfrage teil, wobei mit 42% die Spitäler den Basisversorgung den größten Anteil der Antworten ausmachen. Von diesen 92 betreiben aktuell 51 eine Schmerzambulanz, wobei nur 7 davon im Vollzeitbetrieb laufen. Wien ist mit 12 aktiven Schmerzambulanzen führend. \r\nBei 10 Kliniken stieg die Auslastung in den letzten drei Jahren. Es zeigte sich, dass nun die längste Wartezeit bei bis zu 75 Tagen liegt. \r\nNeun der Befragten gaben an, dass die Schmerzambulanz in den letzten Jahren unter anderem wegen fehlenden personellen und finanziellen Ressourcen geschlossen wurde. \r\nSchlussfolgerung\r\nSollte sich dieser Negativtrend auch in den kommenden Jahren weiter fortsetzen, so könnte es zu einer weiter zunehmenden Verschlechterung der PatientInnenversorgung und -zufriedenheit kommen.\r\nZusätzlich nimmt die Arbeitsbelastung der österreichischen Schmerzambulanzen bei weniger verfügbarem qualifizierten Personal zu, wodurch die Qualität der einzelnen Konsultationen der chronischen SchmerzpatientInnen leiden könnte.\r\nBei fehlenden zeitnahen Veränderungen im Bereich personeller und finanzieller Ressourcen zur Verbesserung der Betriebszeiten und Besetzung der Schmerzambulanzen, sowie der Zufriedenheit der MitarbeiterInnen, wird es zu weiteren Schmerzambulanzschließungen kommen und dies zu einer zunehmenden Belastung der noch praktizierenden SchmerzmedizinerInnen führen. Dadurch kommt es zu einer weiteren Verschlechterung der Versorgung von SchmerzpatientInnen in Österreich. Als Ursache dieser Infarkte findet sich zumeist eine Kleingefäßerkrankung, deren Pathophysiologie noch nicht vollständig geklärt ist. Das Ziel dieser Diplomarbeit ist es, den Langzeitverlauf nach stattgehabtem RSSI genauer zu erforschen. Als Hauptzielgröße wird das Auftreten von zerebrovaskulären und anderen vaskulären (Rezidiv-)Ereignissen definiert. \r\n\r\nMethoden: Insgesamt wurden medizinische Daten aus dem steirischen Krankenhausinformationssystem MEDOCS von 331 Patient*innen, die alle zwischen 01.01.2008 -05.02.2013 an der Universitätsklinik für Neurologie in Graz aufgrund eines RSSIs behandelt wurden, retrospektiv ausgewertet. Danach erfolgte zusätzlich eine telefonische Kontaktaufnahme, bei welcher mittels eines vorher erstellten Fragebogens weitere Informationen über das funktionelle neurologische Outcome (mRS), den derzeitigen Gesundheitszustand sowie die Therapiecompliance gewonnen werden sollten. Die erhobenen Daten wurden anschließend deskriptiv und mittels einfacher explorativer statistischer Verfahren mit der Statistik-Software SPSS (Version „IBM SPSS Statistics 27“) ausgewertet.\r\n\r\nErgebnisse: Die durchschnittliche Zeitspanne zwischen dem Indexschlaganfall und dem Datum des Follow-Ups (aktuellster Arztbrief) betrug 6,4 Jahre. Bei insgesamt 54 Patient*innen der Kohorte konnten keine brauchbaren Follow-Up Daten im MEDOCS-System gefunden werden. Von den 277 übrigen Patient*innen waren 76,1% (n=186) männlich und 32,9% (n=91) weiblich. Der mit Abstand häufigste vaskuläre Risikofaktor im Kollektiv war der arterielle Hypertonus. Insgesamt ist es bei 41,5% (n=115) der Kohorte zu mindestens einem (zerebro-) vaskulären Rezidivereignis gekommen. Von multiplen Ereignissen waren 14,4% (n=40) betroffen. Das mit Abstand häufigste (zerebro-) vaskuläre Reevent war der ischämische Schlaganfall. Insgesamt haben 19,5% (n=54) der Patient*innen mindestens einen ischämischen Rezidiv-Schlaganfall erlitten. Ischämische zerebrale Rezidive traten häufiger bei Patient*innen mit Diabetes Mellitus und hohen HbA1c-Werten auf. Hohe HbA1c-Werte konnten ebenfalls mit dem Auftreten von intrakraniellen Blutungen in Verbindung gebracht werden. Bei der Telefonumfrage mit insgesamt 68 Teilnehmer*innen wurde der derzeitige subjektive Gesundheitszustand auf einer Skala von 0-100 durchschnittlich auf 74 geschätzt und der Mittelwert der Ergebnisse der „modified Rankin Scale“ (mRS) lag bei 1.\r\n\r\nSchlussfolgerung: Die Ergebnisse dieser Arbeit zeigen, dass das Risiko für (zerebro-) vaskuläre Rezidivereignisse nach stattgehabtem RSSI nicht zu unterschätzen ist. Eine gute Einstellung der vaskulären Risikofaktoren und regelmäßige Kontrollen der Laborparameter sowie der Therapiecompliance sind von großer Bedeutung für die Prävention von vaskulären Reevents.
