The effect of checkpoint inhibitors depends on many factors such as presence of immune cells in tumor metastases and tumor intrinsic properties. We investigated the association between metastatic site and outcome in patients treated with immune checkpoint inhibitors.\n\n\nMaterial and Methods:\nWe retrospectively analyzed 103 patients from the Austrian cohort of the Alpine Tumor Immunology Registry (Alpine-TIR), with metastatic NSCLC (72 patients), metastatic RCC (16 patients) and metastatic melanoma (15 patients).\n\n\nResults:\nAmong those patients 24.3% had bone, 20.4% liver, 31.1% lung, 17.5% suprarenal gland and 7.8% brain metastasis, respectively. The median overall survival from diagnosis to death for all patients was 52.3 month. In the univariate cox analysis, lung metastases (hazard ratio: 2.384, 95% confidence interval: 1.160 4.899; p=0.018) correlated with a significantly shorter overall survival. With overall survival calculated from commencing checkpoint inhibitor treatment to death, also liver metastases (hazard ratio 2.374, 95% confidence interval: 1.114 – 5.062; p=0.025) showed a significantly poorer prognosis. Other metastasis location such as suprarenal gland, bone and lymph nodes did not show a correlation.\n\n\nConclusions:\nIn conclusion, in the presented cohort, the efficacy of checkpoint inhibitors is dependent on the metastatic location. The existence of liver metastasis was associated with reduced overall survival from start of checkpoint inhibitor therapy, while lung metastasis showed a significantly poorer overall survival from diagnosis. Understanding the composition of the microbiome, how it interacts with its environment and especially with the host has been the main concern for many publications in the field of microbiome research. \nThe main goal of this thesis was to address some of the questions regarding the interactions of the microbiome with the host and the environment, therefore we tried answering the following questions: Could the environment be a source of human-associated anaerobic microorganisms? Are anaerobic microorganisms able to survive in an aerobic environment? Is preterm labour linked with any changes in the vaginal and urinary microbiome or changes in the human milk oligosaccharides’ (HMOs) composition? Are there any associations between human milk oligosaccharides and the microbiome in urine and vagina? Are the microbiome universal approach methods sufficient to determine the archaeal communities within the human body? What is the composition of the archaeal communities in different body sites? Samples from all projects were analysed by different molecular methods, namely microbial amplicon sequencing and quantitative PCR, and some project-specific methods such as fluorescence in situ hybridization, oxygen tolerance test, and human milk oligosaccharides analysis. \nIn the house microbiome project, we explored the composition of the microbial communities found on the bathroom floor and distinguished between free DNA and DNA obtained from cells with intact cell wall or membrane. By analysing both the bacterial and archaeal communities, we identified that most of the microorganisms present on the bathroom floor are often associated with the human skin or the gastrointestinal and genitourinary tract. Anaerobic microorganisms were also detected, even though the relative abundance of these microorganisms was decreased in the samples which contained DNA only from intact cells. Additionally, we showed that the methanogens present on the bathroom floor are of human origin and that methanogens could survive in an aerobic environment up to 24h. Therefore, the indoor environment could act as a source of anaerobic microorganisms. \nThe results of the UMIC project showed that specific microorganisms found in the vaginal and urinary microbiome were associated with preterm labour, short cervix and preterm birth. Moreover, we observed associations between the sialylated HMOs, in particular, 3’sialyllactose, with preterm birth, high inflammation and short cervix, and confirmed that HMOs influence the microbiome profile. \nIn the last project, the Human Archaeome, we demonstrated that the often-used universal approaches are unable to detect the archaeal communities in human samples, indicating the need of an archaeal-targeting approach for future microbiome studies. Also, we reconfirmed the body site specificity of the archaeal communities. Most body sites share specific taxa such as Methanobrevibacter, Methanobacterium and Methanosphaera, although some body sites have a high microbial diversity, especially the nasal cavity. \nThis thesis expended the knowledge of microbiome regarding indoor environments, the microbiome composition in pregnancy and especially in women at high risk of preterm birth and the human archaeome. Akuter Schmerz, durch Verletzungen, Erkrankungen oder notwendige medizinische Eingriffe, gehört zu den negativsten Reizen, die Kinder erleben (American Academy of Pediatrics. Committee on Psychosocial Aspects of Child and Family Health 2001). In der Schmerzwahrnehmung generell, und somit auch in der postoperativen, spielen neben physischen auch emotionale und psychologische Komponenten eine wichtige Rolle (Joestlein 2015, Lovich-Sapola et al. 2015). Kinder mit stärkerer postoperativer Angst zeigen stärkere postoperative Schmerzen und vice versa (Caumo et al. 2000, Ericsson et al. 2006). Diese gegenseitige Beeinflussung postoperativer Schmerzen und postoperativer Angst konnte in den Studien von Caumo et al. und Ericsson et al. bereits gezeigt werden, wobei in beiden Studien jeweils nur ca. 90 Kinder in die Auswertung einbezogen wurden. Bei Ericsson et al. wurden nur Kinder zwischen 5-15 Jahren mit Z.n. Tonsillektomie untersucht, bei Caumo et al. nur Kinder zwischen 7-13 Jahren (Caumo et al. 2000, Ericsson et al. 2006). Ziel dieser Studie war es, herauszufinden ob sich dieses Phänomen auch für eine größere Studienpopulation für Kinder von 4-18 Jahren im Aufwachraum bestätigt, unabhängig von der Art der Operation, und ob es dabei Unterschiede je nach Schmerzlokalisation gibt.\nMETHODEN:Bei der vorliegenden Studie handelt es sich um eine prospektive Beobachtungsstudie, die im November 2017 ein positives Ethik-Votum erhielt. Nach dem Einholen der Einverständniserklärung der Eltern, wurden Kinder und Jugendliche im Alter zwischen 4-18 Jahren am Tag der Operation zu ihren Schmerzen- sowohl im operierten Bereich als auch zu Halsschmerzen und Schmerzen an einer anderen Stelle- befragt. Angst wurde einerseits mit einem Item abgefragt, aber es wurden auch Aspekte erfragt, die mit Angst assoziiert sind (Nervosität, Zappeligkeit oder Wohlfühlen). In einem Zeitraum von insgesamt 6 ½ Monaten wurden 415 Kinder befragt von denen 411 in die Auswertung eingeschlossen wurden.\nERGEBNISSE:Von den 411 Kindern, die in die Studie einbezogen wurden, waren 153 Kinder (37,2%) weiblich und 258 Kinder (62,8%) männlich. Im Mittel waren die Kinder 11,1 (SD: 4,0) Jahre alt. Bei dieser Studie hatten Kinder, die angegeben haben im Aufwachraum ängstlich gewesen zu sein, signifikant mehr Schmerzen im operierten Bereich, aber nicht mehr Halsschmerzen oder Schmerzen an einer anderen Lokalisation. Nervosität und Zappeligkeit, als Verhalten, das mit Angst assoziiert ist, wurde auch ausgewertet. Hierbei ergaben sich bei allen drei Schmerzlokalisationen keine signifikanten Unterschiede zwischen nervösen und nicht-nervösen Kindern bzw. zappeligen und nicht-zappeligen Kindern. Kinder, die sich „nicht wohl gefühlt haben“ bzw. die sich „nicht wohl fühlten“, als weitere Items zur Einschätzung der Angst, zeigten signifikant mehr Schmerzen im operierten Bereich und an anderen Lokalisationen, jedoch nicht signifikant mehr Halsschmerzen. Kinder, die sich „nicht gut gefühlt“ haben, zeigten bei allen abgefragten Schmerzlokalisationen signifikant mehr Schmerzen.\nCONCLUSIO\nDie gegenseitige Beeinflussung postoperativer Angst und postoperativer Schmerzen, die in anderen Studien gezeigt wurde, konnte in dieser Studie für eine erweiterte Altersgruppe und mit einer Unterscheidung je nach Schmerzlokalisation für den Aufwachraum nur teilweise bestätigt werden (Caumo et al. 2000, Ericsson et al. 2006). Die Überlebenswahrscheinlichkeit einer betroffenen Person kann von einem Laien, der dieses Ereignis beobachtet, um 25 bis 50 % gesteigert werden. \nDie Ergebnisse von Zweiker et al. (2014) über den Wissensstand der Grazer Bevölkerung waren nicht zufriedenstellend, so konnten 36 % der Befragten keine adäquaten Reanimationsmaßnahmen angeben. Das Projekt „Drück Mich!“ wurde daraufhin initiiert und sollte die Bevölkerung über die Laienreanimation aufklären und das Wissen darüber verbessern. \n\nMethoden: Es wurde die Fragebogenuntersuchung wiederholt, um den Wissensstand über Laienreanimation nach dem „Drück Mich!“-Projekt erneut zu erheben. Es wurden 632 Personen am gleichen Ort, dem Grazer Hautbahnhof, befragt. \n\nErgebnisse: In der Befragung 2018 hatten 75,3 % der Passantinnen und Passanten eine Herzdruckmassage spontan assoziiert, wenn sie eine Person mit Herz-Kreislauf-Stillstand vorgefunden hätten. Das ist eine signifikante Steigerung (P-Wert = <0,001) im Vergleich zur Voruntersuchung. 2014 hätten nur 56,3 % an eine Herzdruckmassage gedacht. In der Situation hätten im Jahr 2018 94,5 % die Rettung oder Hilfe gerufen, 2014 hingegen nur 88,7 % (P Wert = 0,016). An eine stabile Seitenlage hätten 2018 15,6 % gedacht, 2014 noch 28,2 % (P Wert = 0,001).\n\nDiskussion: Es konnten verbesserte Kenntnisse der Bevölkerung über die Laienreanimation festgestellt werden. Inwieweit dieser Wissenszuwachs nur durch die Initiative „Drück Mich!“ verursacht wurde, oder auch durch andere Initiativen und Maßnahmen, kann nicht geklärt werden.
