            "title": "Implizites Lernen und Darmmikrobiom bei PatientInnen mit Anorexia nervosa",
            "abstract": "Einleitung: Anorexia nervosa (AN) ist die psychische Erkrankung mit der höchsten Mortalität und weist zudem aktuell nur wenige Therapiemöglichkeiten auf. Es konnte durch Studien bereits gezeigt werden, dass das Darmmikrobiom bei AN verändert ist und die PatientInnen eine verminderte implizite Lernfunktion aufweisen. Dennoch ist bisher nicht geklärt, ob und wie das implizite Lernen mit dem Darmmikrobiom zusammenhängt. In dieser Arbeit wird untersucht, ob es eine Korrelation zwischen einem veränderten Darmmikrobiom und einer verminderten impliziten Lernfunktion gibt und ob hierbei spezielle Bakterien-Taxa eine Rolle spielen.\r\nMethoden: Die für diese Arbeit benötigten Daten wurden im Rahmen der ILSD-Studie erhoben. Die teilnehmenden Personen wurden in zwei Gruppen geteilt: eine Untersuchungsgruppe mit 16 PatientInnen, die an AN erkrankt sind, und eine Kontroll-gruppe mit 14 gesunden Personen. Alle ProbandInnen führten beim Weather Prediction Task (WPT) sowohl die Feedback-Variante (FB) als auch die paired-associated (PA) Variante aus. Die Analyse des Darmmikrobioms erfolgte durch eine Sequenzierung von Stuhlproben mittels Illumina. Anschließend wurde eine bioinformatische Analyse der Daten durchgeführt, um Indices der Alpha-Diversität sowie die Beta-Diversität zu erhalten. Abschließend wurden zwei linear discriminant effect size (LEfSe) Analysen durchgeführt, um statistisch relevante Stämme innerhalb der Gruppen zu identifizieren. Hierbei wurden zum einen die obengenannten Gruppen untersucht und zum anderen Gruppen, welche durch die Zuordnung der ProbandInnen in eine Gruppe mit guter impliziter Lernfunktion und eine Gruppe mit verminderter impliziter Lernfunktion zustande kamen. \r\nErgebnisse: Es konnte eine signifikante positive Korrelation zwischen dem FB-Score und dem Chao-1, den observed species und dem Shannon-Index festgestellt werden. In der ersten LEfSe Analyse konnte für die Untersuchungsgruppe eine signifikante Erhöhung der Spezies Akkermansia muciniphila nachgewiesen werden. Die zweite LEfSe Analyse zeigte eine signifikante Erhöhung der Taxa Firmicutes in der Gruppe mit verminderter impliziter Lernfunktion. Zudem zeigte die Gruppe mit gutem implizitem Lernen eine signifikante Erhöhung der Actinobacteria.\r\nDiskussion: Es konnte eine Korrelation zwischen einer verminderten impliziten Lernfunktion und einem veränderten Darmmikrobiom gefunden werden. Zusammenfassend zeigt diese Studie, dass eine weitere Erforschung des Einflusses des Mikrobioms auf die Lernfähigkeit des Gehirns und deren Auswirkungen notwendig ist.",

            "title": "Prognostischer Nutzen der 18F-FDG PET/CT bei akutem spinalem Trauma. Eine Pilotstudie.",
            "abstract": "Wissenschaftlicher Hintergrund\r\nDie Diffusionsgewichtete Magnetresonanztomografie (DTI-MRT) gilt als Goldstandard für die Darstellung von Weichteilstrukturen und wird effektiv zur Diagnostik und Quantifizierung von Rückenmarksläsionen eingesetzt. PatientInnen mit akuten traumatischen Verletzungen der Wirbelsäule leiden jedoch häufig an einer Instabilität auf Höhe der Verletzung, welche mit chirurgischen Implantaten stabilisiert werden muss. Diese Metallimplantate führen zu Artefakten, die die Bildqualität erheblich verschlechtern und daher die postoperative Beurteilung des Operationsgebietes mittels Magnetresonanztomografie (MRT) erschweren. Es wurde eine alternative Bildgebung mittels 18F-FDG PET/CT hinsichtlich ihrer Eignung zur Beurteilung von Rückenmarksschäden untersucht. Hierbei ergibt sich kein Konflikt durch Metallimplantate. \r\n\r\nMethodik\r\nDie vorliegende Pilotstudie stellt den Glukosestoffwechsel und die funktionelle Neurologie der betroffenen PatientInnen mit Rückenmarkstrauma in Zusammenhang. Diesbezüglich gab es bereits Tierversuchsstudien an Ratten und Hunden. Diese zeigen vielversprechende Ergebnisse und die Möglichkeit einer besseren Quantifizierung der Rückenmarksläsion mit Hilfe der 18F-FDG PET/CT -Untersuchung. Nun wird erstmalig die 18F-FDG PET/CT zur Beurteilung von Wirbelsäulentraumata an Menschen untersucht. Es handelt sich um eine Pilotstudie, in die 9 PatientInnen ab 18 Jahren mit akuter traumatischer Rückenmarksverletzung eingeschlossen wurden. Die PatientInnen wurden an der Univ.-Klinik für Orthopädie und Traumatologie Graz behandelt und im Rahmen ihres stationären Aufenthalts unter strengen Auswahlkriterien in die Studie inkludiert. Nach erfolgter operativer Versorgung und Abschluss der Akutbehandlung wurde eine 18F-FDG PET/CT -Untersuchung durchgeführt und der ASIA-Score erhoben. Im Anschluss erfolgte die Datenauswertung und Niederschrift. \r\n\r\nResultate/Konklusion\r\nIn der Datenanalyse zeigt sich eine signifikante Korrelation zwischen dem ASIA-Gesamtscore und dem SUV-Verhältnis der Läsion zu Cranial sowie zwischen dem Verhältnis SUV Cranial zu Caudal und dem ASIA-Score vor Korrektur. Dies deutet darauf hin, dass eine 18F-FDG -Anreicherung möglicherweise mit dem neurologischen Status der Patienten zusammenhängt. Die Limitationen dieser Pilotstudie sind jedoch vielfältig und schränken die Beurteilbarkeit der Ergebnisse stark ein. Zukünftige Studien sollten größere Fallzahlen, ein ausgeglichenes Geschlechterverhältnis, eine vergleichbare Altersspanne, einheitliche Untersuchungszeitpunkte und ähnliche Lokalisationen der Querschnittsläsionen berücksichtigen. Die vorliegende Studie bildet eine Grundlage weiterer Forschungen zur Korrelation zwischen der 18F-FDG PET/CT und dem ASIA-Score. Der Nutzen der 18F-FDG PET/CT hinsichtlich der Prognose und Therapiekontrolle bei Querschnittsläsion hat sich nicht bestätigt. Weitere Studien sind erforderlich, um diese Zusammenhänge zu validieren und klinische Anwendungen zu prüfen.",

            "title": "Inzidenz von Diabetes und dessen Einfluss auf das Outcome\r\nvon Patient*innen nach Pankreatektomie bei Pankreasmalignomen - die PODIO-Studie",
            "abstract": "Hintergrund\r\nStudienergebnisse zeigen, dass Diabetes mellitus ein Risikofaktor für die Entstehung von Pankreasneoplasien ist und ebenso ein Prädiktor für ein ungünstiges Outcome in dieser Population. Bisher nicht bekannt ist, ob der postoperative Diabetesstatus nach einer pankreatobiliären Tumoroperation Einfluss auf das Überleben nimmt.\r\nMaterial und Methoden\r\nIn dieser retrospektiven Analyse wurden 370 Patient*innen aus dem Operationsregister der klinischen Abteilung für Allgemeinchirurgie der Universitätsklinik Graz inkludiert. Bei den Personen erfolgte zwischen 2016 und 2022 eine Pankreasoperation in benigner oder maligner Indikation. Diese Analyse untersuchte primär den Einfluss einer postoperativen Diabeteserstmanifestation und das damit assoziierte Gesamtüberleben sowie Rezidivrisiko von Personen mit Pankreasmalignomen im Vergleich zu Personen ohne DM. Für die Überlebensanalysen wurden Kaplan-Meier-Kurven, Log-Rank-Tests und Cox-Regression-Modelle herangezogen. Zusätzlich wurden die postoperative Diabetesinzidenz (Typ 3c) und mögliche Risikofaktoren für dessen Entstehung sowohl bei benignen als auch malignen Pankreaserkrankungen analysiert. \r\nErgebnisse\r\nDie postoperative Diabetesinzidenz lag bei 29%, wobei ein höheres Risiko bei malignen (31.3%) verglichen mit benignen (16.7%) Grunderkrankungen bestand (p=0.040, OR=2.28). Personen mit malignen Pankreasneoplasien und postopDM hatten einen signifikant höheren BMI als Personen ohne DM (p=0.029). Eine länger andauernde Operation und ein prolongierter Intensiv- bzw. Normalstationsaufenthalt scheinen das Risiko für eine Diabetesneumanifestation ebenso zu steigern.  Im Vergleich zu Personen ohne DM haben Menschen mit postopDM im 1- und 2-Jahres-Follow-up ein signifikant erhöhtes Risiko zu versterben (HR1-Jahr=2.019 and HR2-Jahre=1.560). Das Rezidivgeschehen bei malignen Pankreaserkrankungen wurde durch den Diabetesstatus in dieser Analyse nicht beeinflusst.\r\n\r\nSchlussfolgerung\r\nEin postoperativ neu-aufgetretener Diabetes mellitus scheint das Outcome von Patient*innen mit Pankreasmalignomen negativ zu beeinflussen. Die Ergebnisse verdeutlichen die Notwendigkeit von prospektiven Studien in größeren Populationen zu dieser Thematik. Zudem bleibt es offen, ob dieser Diabetes ein Marker für eine schlechtere Patientenverfassung ist, beziehungsweise ob der postoperative glykämische Status ein modifizierbarer Faktor ist.",

            "abstract": "Nach wie vor stellt Krebs die medizinische Forschung vor eine große Herausforderung. Das Auftreten von Malignomen nimmt seit Jahrzehnten zu und es wird kontinuierlich nach weiteren Therapieansätzen gesucht. Über 30 Jahre ist es mittlerweile her, dass die Idee der mRNA-Impfung geboren wurde und auch die ersten erfolgreichen Versuche zur Durchführbarkeit dieser Methode liegen schon lange zurück. Trotz zahlreicher vielversprechender Studien schaffte diese Therapieform erst im Kampf gegen die COVID19-Pandemie ihren Durchbruch. \r\nDie mRNA-Impfung bietet im Kampf gegen den Krebs ein hervorragendes Werkzeug für die Realisierbarkeit einer Multi-Target-Therapie, da die Vorlage jedes beliebigen Proteins herstellbar und kombinierbar ist. Diese Therapieform hat sich erfolgreicher als eine Monotherapie bezüglich Tumoreradikation und Resistenzausbildung gezeigt, jedoch sollte die Zielgensuche sorgfältig ausgewählt werden. Das Gen sollte optimalerweise im Tumorgewebe hoch exprimiert werden, im physiologischen Gewebe jedoch kaum bis gar nicht, um die Angriffsfläche zum größten Teil auf die neoplastischen Zellen richten zu können und die Nebenwirkungen möglichst gering zu halten. \r\nDas Ziel dieser Arbeit war die Identifikation von potenziellen Zielgenen für eine Therapie im Bereich der Hämatoonkologie. Hierfür wurden Informationen zu der physiologischen Funktion und Expression der ausgewählten proteincodierenden Gene gesammelt und es wurde sich ein Überblick verschafft, inwieweit eine Expression in Tumoren festgestellt werden konnte und welche Rolle sie hier übernehmen.",

            "title": "Differential expression of miRNAs in A549 cells and plasma samplesfrom lung adenocarcinoma patients and healthy donors",
            "abstract": "MicroRNAs (miRNAs) are small non-coding RNA molecules, which take part in gene regulation on the post-transcriptional level. They are involved in different biological processes,\r\nbut also in tumorigenesis. In this thesis, we wanted to extend our knowledge about miRNA expression and its role in lung cancer, to find out a potential circulating miRNA that could be promising as a diagnostic biomarker for early lung cancer detection. In the first part of the thesis, we aimed to analyze the global expression of 752 miRNAs in vitro in A549 lung adenocarcinoma (LUAD) cells and primary, non-malignant bronchial epithelial (BE) cells from healthy donors, and to identify the most prominently deregulated miRNAs. Expression of 752 miRNAs in LUAD and BE cells was assessed by reverse transcription-quantitative polymerase chain reaction (RT-qPCR) with the mean-centering restricted (MCR) normalization. Out of 752 miRNAs, 37 miRNAs were significantly deregulated in A549 cells compared to BE cells. After setting ΔΔCt ≥ 2.5, eighteen significantly deregulated miRNAs were chosen, 8 were up- and 10 down-regulated in A549 cells compared to BEC cells. Based on those in vitro data, the second part of this thesis aimed to analyze miRNAs expression in plasma samples from patients\r\nwith histologically diagnosed lung adenocarcinoma and healthy donors (n = 18 for each group, total n = 36). We also aimed to identify stably expressed miRNAs in plasma. We found four significant deregulation miRNAs in plasma samples (miR-15b-3p, miR-148a-3p, miR-193b-3p, and miR-195-5p) in LUAD patients compared to donors’ samples. One miRNA (miR-195- 5p) was up-regulated and three miRNAs (miR-15b-3p, miR148a-3p, and miR-193b-3p) were down-regulated in plasma sample of LUAD patients compared to healthy donors. Two miRNAs were used for normalization (miR-16-5p and miR-191-5p), because of their known and proven stability. Finally, by different in silico tools, we identified the target genes influenced by deregulated circulating miRNAs and determine their assignments to specific pathways relevant to the biology of lung adenocarcinoma. Based on the literature, 107 validated genes are regulated by these four miRNAs. Out of 107 validated target genes, five (PTEN, CXCR4, IGF1R, FGF2, and PD-L1) were described to be regulated with more than one of four deregulated miRNAs. Concerning their molecular function, 107 direct target genes were annotated to five main functional groups: binding, catalytic activity, molecular function regulator, molecular transducer activity, and transporter activity. In conclusion, we could demonstrate the relevant differences between cancer and control miRNA expression in vitro and plasma samples of LUAD patients compared to healthy donors. These four dysregulated miRNAs are promising as diagnostic biomarkers for lung adenocarcinoma.",

            "title": "The diagnostical approach of globus sensation: Results from a single institution survey",
            "abstract": "Introduction: Globus pharyngeus, a persistent sensation of a painless foreign body in the throat, drives numerous visits to otorhinolaryngology clinics. The condition's unclear etiology and connections to other ailments hinder the establishment of standardized investigation and treatment approaches. This study, conducted between 2014 and 2019 at the phoniatrics outpatient ward of the University Hospital of Graz, aimed to evaluate diagnostic pathways, findings, and follow-up for patients presenting with globus pharyngeus.\r\nMethods: A retrospective cross-sectional study collected epidemiological data, diagnostic and treatment pathways, and follow-up details for 211 patients identified through the local medical institute for statistics and the \"openMEDOCS\" database. Data analysis involved Microsoft Excel, using descriptive statistics while ensuring patient anonymity.\r\nResults: Among the 211 participants (40.8% male, 59.2% female; mean age 53.5), 79.6% reported chronic complaints. Active smokers accounted for 12.3%, and 26.1% had allergies. The primary complaint was a sensation of a foreign body (64.5%), while the main secondary complaint was burning associated with reflux (20.4%). Flexible laryngoscopy revealed findings in 31.3% of cases, with hyperplasia at the base of the tongue (12.8%) being the most prevalent. The primary recommended post-presentation examination was X-ray or CT/MRI of the neck (46.4%).\r\nConclusion: The collected data were used in conjunction with the current body of research to develop a diagnostic algorithm. By considering potential risk factors (such as a positive smoking history) and warning symptoms, unnecessary examinations are intended to be reduced, thereby alleviating the suffering of patients and relieving the healthcare system as a whole.",
