To avoid allograft rejection, KT comes at the cost of chronic immunosuppression, which harbors an increased risk of infection. Current immunosuppressive treatments are primarily targeted towards T cells. B cells (BCs) have historically been disregarded or reduced to their, potentially detrimental, role as alloantibody producers, but BCs may also provide humoral immunity and exert antibody-independent functions with relevance in transplantation. The COVID-19 pandemic has posed a dual threat for kidney transplant recipients (KTRs), as they are particularly vulnerable to severe courses, while antibody response to vaccination has been suboptimal. In this study, we investigated the changes of BCs and BC subpopulations from before to one year after KT. Additionally, we screened for potential predictors within the peripheral BC pool for vaccination response.\r\n\r\nMethods: A total of 105 KTRs were screened, and blood samples were taken prior to the hemodialysis session before transplantation (T1) and one year after transplantation (T2). Due to incomplete follow-up, death or graft loss 71 KTRs were included in our T1/T2 cohort. Peripheral blood mononuclear cells (PBMCs) were isolated at both timepoints and stained for flow cytometric analysis of leucocytes and BC subpopulations using the markers CD19, IgM, IgD, CD24, CD27, CD86, CD5 and CD38. For the vaccination subcohort, we analyzed antibody response to mRNA-based SARS-CoV-2 vaccination (mRNA-1273 or BNT162b2) in 40 out of 71 KTRs, for whom this data was available. Spearman rank-based correlation between BC subpopulations at T2 and antibody titers were calculated. \r\n\r\nResults: From T1 to T2, leucocyte numbers increased by virtue of monocyte and granulocyte expansion while lymphocytes remained stable. CD19+ BCs trended for increased abundance at T2. Naïve BCs, the largest BC subset, were found at similar frequencies and absolute numbers at both timepoints. Plasmablasts and transitional BCs (TrBCs) were profoundly reduced at T2, whereas double negative BCs increased in abundance. In the memory BC (mBC) pool, IgM-only mBCs and class-switched mBCs were reduced at T2. IgD-only mBCs and unswitched mBCs remained similar. Diminished frequencies and counts of activated CD86+ and tolerogenic CD27- CD5+ BCs were observed at T2. \r\nOnly 50% of KTRs (n= 20) developed detectable antibody titers after two doses of SARS-CoV-2 vaccination. Antibody levels correlated with TrBC counts and frequencies at T2. Responders displayed increased TrBC abundance compared to non-responders without any detectable antibodies. \r\n\r\nConclusion: Flow cytometric phenotyping of circulating BCs revealed profound compositional changes from the CKD G5 to the stable KT setting. Particularly small subsets like PBs and TrBCs were decreased after transplantation. By showing that TrBCs correlated with antibody levels after SARS-CoV-2 vaccination, we provide evidence that quantitative and qualitative changes of the BC pool may be of clinical relevance. Non-frontline workers in the health sector (WHS) were in many cases allowed to work from home (WFH). Changes in work locations have affected the perception of productivity during the COVID-19 pandemic compared to the pre-pandemic perception. Studies regarding this research field are rare for WHS. The aim of the present study was to investigate the perception of productivity and its impact on symptoms of depression during the COVID-19 pandemic. The second objective was to assess the implications for post-pandemic work settings such as WFH or work scenarios in hospitals during pandemics.\r\n\r\nMethods: At three points in time during the COVID-19 pandemic (t1; n = 161: April 2020, t2; n = 1598 winter 2020/2021, t3; n = 1879 winter 2021/2022), an online survey of WHS (e.g., medical doctors, nurses, scientific staff) in Austria concerning their productivity in their current workplace (pre- and post-pandemic) was conducted. The online survey included questions about the perceptions of productivity changes (i.e., perceptions of lower, equal, and higher productivity, before and during the COVID-19 pandemic) in different work settings (e.g., working in a hospital or working from home), as well as standardized questionnaires like the Patient Health Questionnaire (PHQ-9), assessing symptoms of depression in WHS.\r\n\r\nResults: χ2 tests showed that WHS working in hospitals experienced significantly fewer fluctuations in their perceptions of productivity than WHS working from home. An analysis of variance (ANOVA) indicated that WHS with a lower perception of productivity tended to have higher self-assessed depressive symptoms.\r\n\r\nConclusion: The possibility of remaining working in the hospital in stressful scenarios like the COVID-19 pandemic might stabilize the feeling of productivity. Moreover, productivity is associated with self-assessed depressive symptoms. Of particular interest is cardiac form of amyloidosis. The clinical presentation of amyloidosis patients varies depending on the specific type of cardiac involvement. While echocardiography is regarded as the gold standard for non-invasive diagnosis of amyloidotic cardiomyopathy, a definitive diagnosis of amyloidosis typically necessitates a biopsy of the affected organ. In recent times, amyloid-scintigraphy using 99mTc- pyrophosphate (PYP) has emerged as a valuable clinical tool for cardiac imaging in amyloidosis.\r\nAim: The aim of this study is to assess both the diagnostic and prognostic value of amyloid scintigraphy utilizing 99mTc-pyrophosphate in suspected cardiac amyloidosis.\r\nMethods and Materials: A retrospective analysis was conducted, involving 150 individuals who were suspected of having cardiac amyloidosis. Between July 2018 and June 2022 amyloid scintigraphy with 99mTc-pyrophosphate was performed. The results obtained from the amyloid scintigraphy (the Perugini score ranging from 0 to 3, heart-to-lung ratio (H/CL ratio)) were compared with the outcomes of myocardial biopsy and/or cardiac MRI. The patients were categorized into different groups based on the Perugini Score (0, 1, 2, 3) and H/CL ratio (greater or less than 1.5). The median follow-up time was 9 months. Additionally, the association between the Perugini score and life expectancy was examined by the long-rank test.\r\nResults: Myocardial biopsies were performed on 16 patients, and 77 patients underwent cardiac MRI. Amyloid scintigraphy with 99mTc-PYP was considered positive in 133 patients (88.7%) based on visual assessment on planar images (Perugini score 1-3). By contrast, amyloid scintigraphy was considered negative in 17 patients (11.3%) (Perugini score 0). In the semi-quantitative assessment, 80 patients (53.3%) had a H/CL ratio > 1.5, and 70 patients (46.7%) a H/CL ratio ≤ 1.5. When using myocardial biopsy as the gold standard, the Perugini score showed a sensitivity of 92% and a specificity of 33%, and when using cardiac MRI as the gold standard, 91% and 15%, respectively. In contrast, the quantitative H/CL ratio demonstrated a specificity of 67% and a sensitivity of 54% using myocardial biopsy as gold standard and 64% and 64%, respectively using cardiac MRI as gold standard.\r\nRegarding the prognostic value of amyloid scintigraphy with 99mTc-PYP, no significant differences in survival between patients with Perugini scores 1-3 and patients with Perugini score 0, as well as between patients with H/CL ratio >1.5 and <1.5 was found.\r\nConclusion: Our study demonstrates the substantial diagnostic sensitivity of 99mTc- PYP scintigraphy, offering a non-invasive diagnostic alternative that complements conventional diagnostic methods like myocardial biopsy and cardiac MRI in the evaluation of patients with cardiac amyloidosis. Patients with Perugini score 0 and a H/CL ratio ≤ 1.5, respectively had no survival benefit compared to patients with Perugini score 1-3 and a H/CL ratio > 1.5, respectively. Various factors have been described to contribute to a higher willingness to use CAM during the disease trajectory. However, to date, no thorough assessment of patterns of CAM use has been performed among Austrian cancer patients. This study explored factors influencing CAM use in Austrian cancer patients, focusing on symptom clusters and demographical characteristics as predictors. Additionally, the study assessed the evidence-informed practices and research literacy among CAM practitioners involved in cancer patient care. Gaining a deeper understanding of their clinical practice and the role of research in it will facilitate better interdisciplinary collaboration between oncologists and CAM practitioners to benefit cancer patients.\r\nMethods:\r\nWe designed two comprehensive cross-sectional studies involving Austrian cancer patients and CAM practitioners from Austria, Germany, the United States of America, Australia, and New Zealand. Patient data encompassed socio-demographic details, clinical characteristics, patterns of CAM use, and symptom assessment. To identify distinct symptom clusters, we performed a latent profile analysis. We then used bivariate and multivariate analysis to identify predictive factors for CAM use. CAM practitioners provided insights into their demographics, clinical practices, information sources, and the role of research in their clinical practice through an international online survey. We used descriptive statistics and qualitative analysis to describe differences between the participating countries.\r\nResults:\r\nAmong the 171 cancer patients, 63.7% utilized CAM, with prevalent therapies including homeopathy, supplements, herbs, massage, and acupuncture. However, only 42.2% of CAM users disclosed their utilization to their treating oncologist. Symptom cluster analysis identified four distinct clusters, with the drowsiness-depression-anxiety cluster showing a 3.83-fold higher likelihood of CAM use. Multivariate analysis revealed no additional predictors beyond socio-demographic factors. Of the 404 CAM practitioners, 62.9% treated cancer patients, predominantly utilizing acupuncture and herbal treatments. Effective management of pain, depression, nausea, and fatigue was perceivably best achieved with acupuncture, while herbal medicine was preferred for addressing constipation, according to our study respondents. CAM practitioners commonly used acupuncture and herbs concurrently with biomedicine for supportive care, aiming to alleviate treatment side effects. However, only 28.0% maintained contact with treating oncologists. Among all respondents, 87.4% were interested in research, and 62.6% stated that research influenced their practice of acupuncture. In general, CAM practitioners rated the importance of research in their daily practice as moderate. However, they still rated experts' opinions as the most reliable information source. We further found country-specific differences concerning the educational background of CAM practitioners, with the most significant proportion of medical doctors performing acupuncture in Austria.\r\nConclusion:\r\nOur study found symptom clusters representing valuable predictors for CAM use among cancer patients. The development of the drowsiness-depression-anxiety cluster may serve as an indicator to reassess the CAM use of patients by oncologists, suggesting continuous monitoring of the symptom burden during the patient’s disease trajectory. However, we also identified a communication gap between oncologists and patients. Promoting proactive patient-centered communication into clinical practice may identify potentially harmful CAM practices and offer the chance to guide patients in their choice of CAM treatments, further strengthening the physician-patient relationship. Current literature establishes a link between endothelial dysfunction and various diseases as well as exogenous factors, that heighten the risk. Notable risk factors include pregnancy induced hy-pertension (PIH), preeclampsia, HELLP-syndrome, gestational diabetes mellitus (GDM), antiphospholipid syndrome (APS), intrauterine growth restriction (IUGR), and infections during pregnancy.\r\n\r\nAims and objectives: Due to the correlation of the diseases with endothelial dys-function, this thesis aims to present the pathophysiology associated with the endo-thelium, with the objective to provide a basis for further research on prevention and treatment.\r\n\r\nMethodology: We conducted a systematic literature review, drawing upon studies identified on PubMed and Web of Science. \r\n\r\nResults: Cases of PIH, APS, IUGR, and certain exogenous factors exhibit reduced endothelial nitric oxide synthase (eNOS) activity and its product nitric oxide (NO). Conversely, eNOS activity is increased in GDM. Both GDM and preeclampsia are linked to heightened mitogen-activated protein kinase pathway activity, disrupting endothelial function. While obstetric infections, APS, and preeclampsia diminish at least one of the parameters - angiogenesis, proliferation, or migration -, IUGR re-sults in a pathological increase.\r\n\r\nConclusion: This review highlights several pathological mechanisms in high-risk pregnancies that could serve as targets for diagnosis and treatment. Hinzu kommen die hohe psychosoziale Belastung und der Einfluss auf die Lebensqualität der Patient*innen (HRQL). Der “Primary Sjögren’s Syndrome Quality of Life (PSS-QoL)” Fragebogen ermöglicht die Erhebung der HRQL mit einem ganzheitlichen Ansatz. \r\nMethoden: Der PSS-QoL von PSS-Patient*innen (n = 104), klinische und laborchemische Parameter aus dem PSS-Register der rheumatologischen Ambulanz der Medizinischen Universität Graz, wurden über einen Zeitraum von bis zu vier Jahren ausgewertet und dargestellt. Weitere Fragebögen und Indices zur Erhebung der Krankheitsaktivität (ESSDAI, ESSPRI, Xerostomia Inventory, Sicca-Score, OSDI) werden betrachtet. Diese Arbeit befasst sich mit der statistischen Auswertung und longitudinalen Vergleichsanalyse der erhobenen Daten. \r\nErgebnisse: Im Zeitraum von bis zu vier Jahren konnte in dieser Arbeit keine signifikante Veränderung der Lebensqualität der Patient*innen beobachtet werden (p > 0,05). Der PSS-QoL Gesamtscore liegt im Mittel bei 33 Punkten. Einzig der PSS-QoL_Trockenheit nimmt im Verlauf signifikant ab (V0M = 9,6 (± 4) vs. V36M = 6 [0 - 23]; p < 0,001). Patient*innen mit einem Late-Onset (≥ 65 Jahre) haben höhere PSS-QoL Werte. Die immunologische Aktivität ist bei Patient*innen mit Early-Onset höher (≤ 35 Jahre: RF IgA = 500 [1 - 500], IgG = 19,76 (± 6) vs. Late-onset: RF IgA 1 [1 - 102], IgG 12,25 [7 - 37]; p < 0,05). Das Alter zu Beginn der Betrachtung zeigt ähnliche Ergebnisse (Alter ≤ 50 Jahre: PSS-QoL_Gesamt =18,5 [6 - 61]; RFIgA = 500 [1 - 500] vs. Alter ≥ 70 Jahre: PSS-QoL_Gesamt = 34 [12 - 72]; RFIgA = 11 [1 - 500]; p < 0,05). Die Krankheitsdauer von 5 - 10 Jahren zeigt im Vergleich die niedrigsten Werte für den ESSDAI und IgG. Für Patient*innen, die symptomatisch mit Pilocarpin therapiert werden, zeigen sich höhere PSS-QoL Scores (39,8 [9 - 72] vs. 28 [6 – 69]; p = 0,018). Der PSS-QoL korreliert direkt mit Onset, Symptomdauer und Alter und indirekt mit IgG. \r\nDiskussion: Die HRQoL der Patient*innen bleibt konstant. Eine zu kurzer Beobachtungszeitraum, eine zu kleine Stichprobengröße und eine derzeit nur unspezifische Therapie könnten Ursache dafür sein. Der Krankheits-Onset hat sich als relevant herausgestellt. Insbesondere die hohe immunologische Aktivität beim Early-Onset gibt Anlass zu weiteren Diskussionen. Die PSS-Diagnose ist für die Erhaltung der HRQoL von entscheidender Bedeutung und erfordert eine Sensibilisierung in der Ärzt*innen in der Erstversorgung. Das Selbstbewusstsein der Patient*innen mit ihrer Erkrankung wird durch den PSS-QoL gefordert und trägt zur Verbesserung der HRQoL bei. Der PSS-QoL zeigt Potenzial in Studien als Outcome und als Therapieziel Verwendung zu finden. Since, there are some hints in the literature that cannabinoids might have some anti-cancerous effects, cannabinoids are also taken by patients with metastatic disease under conventional therapy. \r\nObjectives: This study addressed the question if and how cannabinoids mediate pro-apoptotic effects in metastatic melanoma in-vivo and in-vitro and its value in comparison with conventional targeted therapy in-vivo.\r\nMethods: Several melanoma cell lines were treated with different concentrations of cannabinoids and anti-cancer efficacy was assessed by proliferation and apoptosis assays. Subsequent pathway analysis was performed using apoptosis, proliferation, flow cytometry, immunohistochemistry, cytokine array and confocal microscopy data. Efficacy of cannabinoids in combination with trametinib was studied in NSG mice in vivo.\r\nResults: Cannabinoids reduced cell viability in multiple melanoma cell lines in a dose-dependent manner. The effect was mediated by CB1, TRPV1 and PPARα receptors whereby simultaneous pharmacological blockade of all three receptors protected from cannabinoid-induced apoptosis. Die weltweite Prävalenz unter Erwachsenen liegt bei 31% (1, 2). Ergebnisse aktueller Studien wie der SPRINT-Studie (3) geben Grund zur Diskussion über die Verwendung niedrigerer Blutdruckziele sowie die Anwendung von unbeobachteten Messmethoden, welche weniger anfällig für systemische Fehler sein könnten.\r\nMethoden\r\nBei 100 Patient*innen mit bekannter arterieller Hypertonie, die zu einem Routinebesuch in die allgemeine kardiologische Ambulanz der Klinischen Abteilung für Kardiologie der Universitätsklinik für Innere Medizin des LKH-Univ.-Klinikum Graz kamen, wurde eine konventionelle, standardisierte 3-fach Office-Blutdruckmessung nach einer 5-minütigen Ruhephase durchgeführt. Der Mittelwert aus der zweiten und dritten Messung wurde für weitere Untersuchungen verwendet. Anschließend wurde eine unbeobachtete automatisierte Office-Blutdruckmessung durchgeführt, wobei hier der Mittelwert aus zwei Messungen mit programmierter Pause von zwei Minuten erhoben wurde. Ziel war es, herauszufinden, wie hoch die Quote der unkontrollierten Hypertoniker in der Population ist und ob es relevante Unterschiede in der Höhe des Blutdrucks zwischen den beiden Messmethoden gibt. Als Messgeräte wurden Boso Medicus System und Omron HEM-907 XL monitor verwendet.\r\nErgebnisse\r\nDer systolische Wert war in der unbeobachteten, automatischen Messung im Mittel um 1,6 (± 0,74 SEM1) mmHg höher (p <0,05), der diastolische Wert jedoch um 3,8 (± 0,56 SEM) mmHg niedriger (p <0.0001) als in der konventionellen Messung. Bei 44% der Teilnehmer*innen der Office-Messung bzw. 42% in der unbeobachteten automatisierten Messung lag der systolische Blutdruckwert über 140 mmHg. Diastolische Werte über 90 mmHg waren in der Office-Messung bei 13%, in der automatisierten Messung bei 11% feststellbar. Nach Bland-Altman wurde die 1,96-fache Standardabweichung der Differenz als obere und untere Grenzlinie verwendet. Die statistische Auswertung des Vergleichs der Messmethoden zeigte, dass die beiden Messmethoden relativ geringe aber signifikante Blutdruckunterschiede ergaben.\r\nSchlussfolgerung\r\nDie mittleren Differenzen der Vergleichsuntersuchung lagen unter den Grenzwerten der Deutschen Hochdruckliga 5 (± 8 SEM) mmHg wonach die unbeobachtete, automatische Messung der Office-Messung gleichgestellt werden kann. In aktuellen Guidelines – z.B. in der 2020 publizierten Leitlinie der Gesellschaft „Hypertension Canada“ – wird diese Messmethoden für Ordinationen empfohlen. Diese Funktion machte das Myokin zu einem vielversprechenden Kandidaten als Ansatzpunkt für mögliche Interventionen bei Krankheiten, die mit Muskelatrophie assoziiert sind. Inzwischen wurden viele verschiedene Wege erforscht, um Myostatin zu beeinflussen und therapeutisch zu nutzen. Die vielfältigen Therapien unterscheiden sich dabei in ihrer Funktionsweise, ihren Nebenwirkungen, ihrem Sicherheitsprofil sowie ihrer klinischen Effektivität. \r\nZiel: Diese Diplomarbeit vergleicht zwei grundlegend unterschiedliche Ansätze der Hemmung von Myostatin: genetische und pharmazeutische Inhibition. Die wichtigsten Therapeutika beider Gruppen werden anhand ihrer Funktionsweise, Nebenwirkungen, Sicherheitsprofile sowie klinischen Effektivität verglichen. Basierend auf diesem Vergleich wird der Stellenwert der Inhibitoren im klinisch-kurativen Setting sowie im Sport bewertet.\r\nMaterial und Methoden: Zur Beantwortung der Forschungsfrage wurde eine Literaturrecherche in den Datenbanken PubMed und Google Scholar durchgeführt. Die Auswahl der wichtigsten Therapeutika erfolgte anhand des Fortschritts der klinischen Forschung. Für 4 Wirkstoffe wurde eine weitere, vertiefende Literaturrecherche vorgenommen. Nach Screening der Abstracts von initial 414 Arbeiten wurden 42 eingeschlossen und für die Forschungsfrage berücksichtigt.\r\nErgebnisse: Die Forschung zu pharmazeutischer Inhibition ist sehr viel weiter fortgeschritten als zu genetischer Inhibition. Der einzige genetische Inhibitor hat nicht genügend Evidenz, um definitive Aussagen zuzulassen. Obwohl er in der bisherigen Studie vielversprechende Ergebnisse bezüglich Sicherheit, Nebenwirkungen und Effektivität zeigt, wurde seit 2017 keine weitere Forschung zu genetischer Inhibition veröffentlicht. Alle untersuchten pharmazeutischen Wirkstoffe hingegen zeigen ein vorteilhaftes Sicherheitsprofil und gute klinische Effektivität, wobei sich die Wirkungen und somit die Anwendungsgebiete unterscheiden. \r\n \r\nDiskussion: Pharmazeutische Inhibitoren sind aktuell den genetischen weit voraus. Mit Luspatercept erhielt ein Wirkstoff sogar bereits klinische Zulassung zur Behandlung des myelodysplastischen Syndroms. Auch zur Behandlung des pulmonalen Hypertonus und von Beta-Thalassämien könnten Myostatininhibitoren bald in den klinischen Alltag Einzug halten. Gesundheitsprofessionist*innen, welche in der direkten Versorgung von Patient*innen tätig sind, werden sich mit großer Wahrscheinlichkeit im Laufe ihres Berufsalltages mit der Betreuung am Lebensende auseinandersetzen müssen. Aufgrund dessen ist es notwendig, dass sie diese Phase erkennen und die damit verbundenen Aspekte und Herausforderungen verstehen, um adäquat handeln zu können.\r\n \r\nZiel: Ziel dieses Scoping Reviews ist es, einen Überblick zu den Lehr- und Lernstrategien, welche die Thematik End of life in die Aus-, Fort und Weiterbildung von Pflegepersonen integrieren, zu geben. \r\n\r\nMethode: Bei dieser Arbeit handelt es sich um ein Scoping Review, in welchem Studien, die zwischen 2002 und 2023 veröffentlicht wurden, inkludiert sind. Eine systematische Literaturrecherche wurde in den Datenbanken CINAHL, EMBASE, Cochrane Central Register of Controlled Trials, ERIC und PubMed durchgeführt. Die in den Studien beschriebenen Lehr- und Lernstrategien wurden extrahiert und anschließend narrativ sowie tabellarisch dargestellt.\r\n\r\nErgebnisse: Es konnten 33 Studien ausfindig gemacht werden, welche Lehr- und Lernstrategien zum Thema End of life in der Aus- und Weiterbildung von Pflegepersonen beschreiben. Die Ergebnisse der Studien konnten zu folgenden Lehr- und Lernstrategien zusammengefasst werden: Interaktives Lernen, Affektives Lernen, Simulation, Blended Learning, Problem Based Learning, Kooperatives Lernen, Interprofessionelles Lernen, Online Learning und Frontalunterricht. \r\n\r\nSchlussfolgerung: Auszubildende profitieren laut eigenen Angaben insbesondere durch den Erfahrungsaustausch mit anderen Pflegepersonen und Berufsgruppen. Our aim was to determine the incidence, clinical consequences, and risk factors for VTE during ICI-therapy to support the development of future thromboprophylaxis strategies.\r\nMethods: Consecutive adult patients treated with ICI at the Medical University of Graz were included in this retrospective cohort study and followed for the occurrence of VTE during ICI-therapy. Statistical analyses were conducted in competing risk analysis, accounting for all-cause mortality as competing event. Risk of VTE according to time dependent exposure to ICI after cancer diagnosis was analysed in a multi-state model. Multivariable adjustment of models was conducted to account for potential confounders. \r\nResults: Overall, 417 patients were included [non-small cell lung cancer (41%), renal cell carcinoma (16%) and melanoma (15%)]. Over a median follow-up of 26.4 months, 37 VTE occurred [cumulative incidence: 12.2% (95% confidence interval [CI]: 8.7-16.4)]. VTE-risk was increased after ICI-initiation compared to the period from cancer-diagnosis to ICI-start (transition-hazard-ratio (HR): 3.30, 95%CI: 1.95-5.57). Similar incidences and no significant differences in risk were observed according to patient demographics, comorbidity burden, cancer-type and -stage. The Khorana-score did not predict VTE-risk (subdistribution-HR [score ≥2]: 0.88, 95%CI: 0.45-1.72). Baseline levels of routine laboratory parameters including C-reactive protein (CRP) did not predict VTE-risk, yet early increases in CRP (2-fold increase within 3 months of ICI-initiation) were associated with a significantly higher VTE-risk (adjusted subdistribution-HR: 2.31, 95%CI: 1.06-5.02), with a cumulative incidence of 22.9% in patients with an early CRP-rise.\r\nConclusion: A substantial burden of VTE among ICI-treated patients was observed, characterised by homogenously high risks irrespective of underlying patient- and cancer-characteristics. Die dazugehörige Österreichische Plattform für Gesundheitskompetenz legt einen ihrer Schwerpunkte auf die „Gute Gesprächsqualität im Gesundheitssystem“. Für den telemedizinischen Bereich liegt auf dieser Plattform aktuell lediglich ein Leitfaden für die Video- und Telekonsultation vor. Im Zuge des interuniversitären Projektes „Digital Skills, Knowledge & Communication“ hat sich die Medizinische Universität Graz auf den Teilbereich „Digitalisierung in der Kommunikation mit PatientInnen“ fokussiert und ihre bereits vorhandene Expertise im Bereich der Kommunikation um den digitalen Aspekt erweitert. Der vorliegende Systematic Review wird im Rahmen dieses Projektes verfasst. Ziel ist es, die aktuelle Evidenz zu Kommunikations-Ausbildungen für Studierende der Gesundheitsberufe, die jene auf das telemedizinische Setting vorbereiten, zusammenzufassen. \r\nMethodik: Die systematische Literatursuche wurde in den Datenbanken PubMed, Ovid (Embase), CINAHL sowie Cochrane Library durchgeführt und durch eine Handsuche auf Google Scholar sowie in den Referenzlisten ergänzt. Es wurde mit ausgewählten Keywords einschließlich MeSH-Termini recherchiert und die vorab festgelegten Ein- und Ausschlusskriterien beachtet. Die Qualität der eingeschlossenen Studien wurde mithilfe der Joanna Briggs Bewertungstools durch zwei Reviewer bewertet. \r\nErgebnisse: Die 15 identifizierten Studien konnten in vier Kategorien unterteilt werden: Die aktuellen telemedizinischen Kommunikationstrainings für Studierende der Gesundheitsberufe werden unter Einbezug von Simulations- bzw. SchauspielpatientInnen, Rollenspielen, einem asynchronen Curriculum und echten PatientInnen vermittelt. Das am häufigsten eingesetzte Lehrmittel in dieser Arbeit sind die Simulations- und SchauspielpatientInnen. Die Ausbildungen werden inhaltlich durch unterschiedliche Themen ergänzt, darunter die Vor- und Nachteile der virtuellen Konsultation sowie die ethischen und rechtlichen Aspekte. \r\nDiskussion: Bestehende telemedizinische Kommunikationstrainings werden größtenteils ohne (beschriebene) theoretische Grundlage umgesetzt. Ein Großteil der Studien richtet sich zudem an Studierende der Medizin, während sich hinsichtlich der Trainings für andere Gesundheitsberufe eine Lücke zeigt. Im Zuge weiterer Forschung sollen Trainingseinheiten mit Kontrollgruppen konzipiert und Interventionsstudien durchgeführt werden. Die Antibiotika-assoziierte Diarrhoe (AAD) ist eine Erkrankung, die im klinischen Alltag häufig zu finden ist. Die Pathomechanismen dafür sind vielfältig und es bedarf weiterer Forschungsarbeit, diese endgültig zu klären. Mit dieser Diplomarbeit soll ein Beitrag zum besseren Verständnis der AAD geleistet werden.\r\nIn der Einleitung finden sich Begriffsdefinitionen und theoretische Grundlagen zum Thema Antibiotika und Diarrhoe, außerdem werden Grundlagen sowie mögliche Einsatzgebiete von Entzündungsmarkern im Stuhl dargestellt.\r\nMaterial und Methoden. Zur Beantwortung der Forschungsfragen „Welche Rolle spielt die fäkale Calprotectinbestimmung bei der Antibiotika-assoziierten Diarrhoe (AAD)?“ und „Welche Rolle spielt die fäkale Calprotectinbestimung nach Antibiotikaeinnahme ohne Diarrhoe?“ wurde eine systematische Literaturrecherche mithilfe der Meta-Plattform „Pubmed“ durchgeführt. Letztlich wurden 21 passende Untersuchungen gefunden, welche in neun Kategorien unterteilt, für die Beantwortung der Forschungsfragen herangezogen werden konnten. \r\nErgebnisse. Die Studien zeigen, dass die fäkale Calprotectinmessung bei der Differenzierung zwischen Clostridioides difficile Infektionen (CDI) und non-CDI AAD, sowie bei der Differenzierung zu anderen Ursachen einer Diarrhoe hilfreich sein kann. Der fäkale Calprotectinwert weist einen Zusammenhang zu auftretenden Komplikationen bei CDI und dem CDI-Schweregrad auf. Eingeschränkt kann der Wert herangezogen werden, wenn es um die Differenzierung von manifester CDI und asymptomatischer Clostridioides difficile (C. diff.) Kolonisation, C. diff. mit oder ohne Toxinproduktion, Prognose von CDI-Rückfällen und dem messbaren Therapieerfolg geht. Die Rolle des fäkalen Calprotectinwertes zur Vorhersagekraft für späteres Therapieansprechen ist fraglich. Ein Zusammenhang von hohen Werten und CDI-bezogenen Todesfällen besteht nicht. Zur Aussagekraft von fäkalen Calprotectinwerten bei Patient*innen nach Antibiotikaeinnahme ohne Diarrhoe finden sich keine Studienergebnisse.\r\nSchlussfolgerung. Fäkale Calprotectinmessungen zeigen für einige Fragestellungen eine hohe Aussagekraft. Bei anderen Fragestellungen zeigt sich hingegen bis jetzt kein eindeutiger Nutzen der Messung. Insgesamt sind weitere prospektive Studien mit großer Teilnehmerzahl notwendig, um die Calprotectin-Bestimmung auch in anderen klinischen Situationen zu etablieren.
