Recurrence of cancer within five years after tumor resection is of significant concern as it is related with a high risk of cancer-associated deaths. Predictive biomarkers are urgently needed. We, therefore, hypothesize that the spatial tissue composition of relapsed and non-relapsed colon cancer stage II patients after tumor resection reveals relevant biomarkers (Sallinger et al., 2023).\r\n\r\nThe spatial tissue composition of stage II colon cancer patients was examined by a novel cutting-edge spatial transcriptomic technology, namely in situ sequencing, with single cell and sub-cellular resolution. A panel of 176 genes was designed to investigate various cancer-associated processes and components of the tumor microenvironment. Based on spatial expression patterns generated by in situ sequencing (GTC-tool – Genes-To-Count), we identified a tumor gene signature to subclassify tissue into neoplastic and non-neoplastic tissue structures. Three differentially expressed genes (FGFR2, MMP11 and OTOP2) in the neoplastic tissue compartments of patients who developed a relapse in comparison to non-relapsed patients were identified, which predict recurrence in stage II colon cancer (Sallinger et al., 2023). Additionally, to confirm the specificity of the used technology, cell-phenotyping of one patient sample was performed by using the in situ sequencing generated data. \r\n\r\nIn conclusion, this thesis highlights the importance of understanding the biological processes that contribute to tumor dissemination and provides insight into the underlying mechanisms. We were the first to apply a direct RNA-hybridization based in situ sequencing approach in colon cancer for the purpose of identifying new potential biomarkers. Die Pathomechanismen der oft mit neuropathischen Komponenten verbunden chronischen Schmerzen sind noch nicht vollständig geklärt. Es existieren jedoch unterschiedliche Ansätze von chirurgischer als auch anästhesiologischer Seite zur Verringerung der Inzidenz bzw. der Intensität des Postthorakotomie-Schmerzsyndroms.\r\nDas Ziel dieses systematischen Literaturreviews ist, anästhesiologische Verfahren zur Prävention chronischer Schmerzen nach Thoraxeingriffen anhand von qualitativ hochwertigen randomisiert-kontrollierter Studien zu identifizieren.\r\n\r\nMethodik\r\n\r\nDen Ausgangspunkt für diesen Review bildete die Suche nach Studien anhand zuvor festgelegter Schlagwörter in den zwei größten medizinischen Datenbanken Embase und Medline. Die Ergebnisse wurden einem Screening-Prozess anhand des\r\nPICO-Schemas unterzogen. Eingeschlossen wurden randomisiert-kontrollierte Studien mit einem Jadad-Score von mindestens 3 Punkten zu präoperativ durchgeführten Interventionen seitens der Anästhesie an Thorakotomiepatient*innen, bei welchen\r\ndie Erfassung der Schmerzwerte mindestens drei Monate nach dem Eingriff stattfand.\r\n\r\nResultate\r\n\r\nAus den insgesamt 380 Suchergebnissen entsprachen 25 randomisiert-kontrollierte Studien zur Prävention des Postthorakotomie-Schmerzsyndroms den Einschlusskriterien\r\ndes Reviews. Es konnten unterschiedliche Ansätze zur Prävention identifiziert werden: Die Verfahren wurden anhand der Art der Intervention in multimodale präventive Analgesie, Art des Narkoseverfahrens, thorakale Periduralanästhesie, regionale Verfahren und präoperative pharmakologische Interventionen gegliedert.\r\nDie Gruppe der pharmakologischen Interventionen hatte dabei den größten Anteil, untersucht wurden vorrangig Gabapentinoide und Ketamin bzw. S-Ketamin. Des Weiteren fand sich zu Dexmedetomidin und Dextrometorphan jeweils eine Studie. Bei den regionalen Verfahren waren neben der thorakalen Periduralanästhesie der Paravertebralblock und der Serratus anterior plane Block vertreten.\r\n\r\nDiskussion\r\nAufgrund der unterschiedlichen Studiendesigns und Outcomes sowie der heterogenen Qualität der einzelnen Studien innerhalb der Gruppen lässt sich die ursprüngliche\r\nFragestellung nicht einfach beantworten. Festzustellen ist jedoch, dass sich für den präventiven Einsatz diverser Pharmaka (v.a. Ketamin und Gabapentinoide) derzeit\r\nkaum Evidenz findet. Hier sind größer angelegte Studien mit einem für das Postthorakotomie-Schmerzsyndrom adäquaten Beobachtungszeittraum von Nöten.\r\nDer in einer Studie durchgeführte Vergleich zwischen totaler intravenöser Anästhesie und inhalativer Anästhesie zeigte ein geringeres Auftreten chronischer Schmerzen in der TIVA-Gruppe – eine pauschale Empfehlung für das Anästhesieverfahren bei Thoraxeingriffen lässt sich daraus jedoch nicht ableiten. Neuraxiale und regionale\r\nVerfahren konnten in den eingeschlossenen Studien einen Vorteil hinsichtlich der Prävention chronischer Schmerzen zeigen.
