Therefore, the present thesis aims at developing a neuroscientifically informed psychodynamic framework regarding the etiology of SUD, in order to path new ways for the understanding and treatment of this potentially life-threatening condition. While traditional psychoanalysis has been criticized as insufficient for the treatment of SUD, recent progress in the field of neuropsychoanalysis has generated new and promising hypotheses regarding its etiology. However, empirical research applying this framework has been sparse. In order to fill this gap, the original research presented in this thesis consists of seven studies investigating connections between childhood trauma, primary emotions, personality structure, attachment and neurophysiological parameters as well as their relation to addiction development and treatment. \nStudies indicate that SUDs are significantly associated with childhood trauma and a variety of psychodynamic personality concepts including insecure attachment, decreased personality organization and decreased primary emotion functioning. In correspondence to this, structural equation modeling and path analysis estimations suggest that the pathogenic effect of childhood trauma is mediated by its relation to impaired personality structure. On a neuroscientific level, these results are paralleled by evidence for neurological alterations associated with SUD, especially pronounced in widespread deviations in white matter fiber tracts and decreased cortical thickness predominately affecting the left insula and lateral orbitofrontal cortex. Furthermore, SUD patients showed decreased oxytocin reactivity in response to an attachment-based intervention.\nThe results not only highlight the empirical validity of the neuropsychoanalytic approach towards SUD etiology but also present tentative evidence for the neurophysiological correlates of psychodynamic concepts. In particular, the findings underscore the conceptualization of addiction as a disorder related to dysfunctional attachment and affect regulation abilities, often caused by traumatic childhood relationships. In turn, these deficits seem to be reflected by alterations in subcortical and neocortical structures as well as reduced oxytocin reactivity. In the last years new diagnostic tests were developed. Galactomannan testing (GM) and Aspergillus PCR have both become essential for diagnosis of IA in patients with haematological malignancies. Whether or not PCR and GM need to be performed in bronchoalveolar lavage fluid (BALF), a sample from the direct site of infection, or testing of blood samples is sufficient, remains unknown.\n\nMethods\nWe evaluated the diagnostic performance of GM ELISA, and Aspergillus specific PCR by using BALF samples and blood specimens obtained at the same day. In case of suspected IA and when a diagnostic bronchoscopy was performed in clinical routine, BALF aliquots were reserved for study purposes and additionally whole blood sample were collected within 24 hours of bronchoscopy. GM testing was performed at the Clinical Institute of Medical and Chemical Laboratory Diagnostics, Medical University of Graz, Austria. BALF aliquots together with whole blood samples, were shipped overnight to the scientific laboratory in Mannheim, Germany for BALF Aspergillus specific and panfungal PCR testing. Additionally, we expand our cohort by including cases from Mannheim, which were enrolled retrospectively.\n\nResults\nA total of 53 immunosuppressed patients were included in the final analysis, with 16 probable/proven IA and 37 with no evidence of IA according to the revised EORTC/MSG criteria. 38 patients with haematological malignancies were prospectively enrolled at the Medical University of Graz, Austria, 15 patients with mixed underlying diseases at the Mannheim University Hospital, Germany. Patients with possible IA were excluded from this analysis. A total of 34/53 (64%) of all patients and12/16(75%) of patients with probable/proven IA received antifungal treatment at the time of the BALF recovery. Sensitivities of Aspergillus PCR and GM were 44% and 38% in BALF, and 0% and 31% in blood, respectively. BALF PCR results demonstrated to be superior (sensitivity 44%, specificity 100%) compared to blood PCR. Combination of BALF Aspergillus PCR, BALF GM (>1.0 ODI), BALF-culture and/or serum GM (>0.5ODI) resulted in the highest sensitivity (75%) for probable/proven IA cases.\n\nConclusion\nIn conclusion sensitivity was low in every evaluated diagnostic test when interpreted on their own (low in BALF and even lower in blood). In our patient cohort diagnostic test combination resulted in a significant higher diagnostic performance and could improve diagnosis and management of invasive aspergillosis. The most accurate and reliable method for measuring subcutaneous adipose tissue (SAT) thickness is the recently developed and standardised brightness mode ultrasound (US) technique; however, the application of this method has not been tested in overweight and obese people before.\nThe aims of this thesis were: \n(a) to contribute to the methodical developments of the standardised US technique by testing the applicability of the method in overweight and obese persons, by investigating the skin thicknesses at the eight standardised sites, and by comparing mean SAT thicknesses of eight sites to means of 216 randomly distributed sites. \n(b) to conduct intra-observer reliability tests in all groups from lean to obese. \n(c) to analyse the participants' body composition characteristics in terms of fat patterning, amounts of fibrous structures embedded in the SAT, and correlations of SAT with anthropometric indices.\n\nMethods: The 82 men and 71 women investigated covered a wide range of SAT thicknesses, and BMI ranged from 17 to 40 kgm-2. The image evaluation software enabled semi-automatic multiple thickness measurements in SAT and quantification of the embedded fibrous structures (fasciae). US measurements were made in accordance with the standardised method at the eight sites: upper abdomen (UA), lower abdomen (LA), erector spinae (ES), distal triceps (DT), brachioradialis (BR), lateral thigh (LT), front thigh (FT) and medial calf (MC). The previously used site external oblique (EO) was replaced by LT. Additionally, in a subset of ten participants, SAT thickness measurements were performed at 216 randomly distributed sites. Skin thicknesses were also measured. Obtainable accuracy of US measurements ranged from about 0.1 mm (at 18 MHz probe frequency) to about 0.3 mm (at 6MHz).\n\nResults: The site EO caused major problems in overweight and obese persons, therefore, the new site LT was introduced instead. Mean skin thicknesses were significantly higher in men compared to women at all sites, except for ES, and the thicknesses were also significantly different at all sites (p<0.05). The means from the eight sites (means of SAT thicknesses ranged from 3 to 10mm) overestimated the means obtained from 216 sites because typical fat depot sites are overrepresented in the set of the eight sites. Within this range of SAT thicknesses, the 216-site mean was 0.65 (SD 0.05) times the mean of the eight sites. \nThe intra-observer study resulted in a correlation coefficient of ρ=0.999, the standard error of the estimate (SEE) was 1.1 mm, and 95% of measurements were within ±2.2 mm. In the subgroup with sums of SAT thicknesses below 77mm, 95% of measurements were within ±1.4mm. Except for BR, all sites showed significant differences in median SAT thicknesses between men and women. SAT thickness sums differed significantly when men (median 40.3mm) and women (median 76.0 mm) were compared. Significant sex differences in the amounts of fibrous structures were found at the individual sites ES, DT, LT, FT, MC, and also for the sums of the eight sites (15%, and 9%, respectively).\n\nConclusion: In a wide range of body fatness from extremely lean to obese, high reliability can be expected when using the standardised US method for measuring SAT. The thickness measurement deviations increased with increasing SAT thickness, but the percentages of deviations (with respect to the given SAT thickness) decreased. Pronounced dimorphisms between men and women was found for the following variables: sums of SAT thicknesses, skin thicknesses, amount of fibrous structures embedded in SAT, and in the fat patterning. Excessive protein and messenger ribonucleic acid (mRNA) levels of GIRK1 have been found in ER+ breast cancer samples. They are associated with lymph node metastasis and reduced the probability of survival of patients. GIRK1 poses a promising target for the prognosis and therapy of breast cancer. Substantial expression of GIRK1 mRNA and protein also occurs in several cell lines cultured from breast tumors. The malignant MCF7 breast cancer cell line expresses GIRK1 mRNA already at moderate levels. Therefore, this cell line provides an excellent model for increased GIRK1 expression in breast tumors. Ectopic hyperexpression of GIRK1 in MCF7 cell lines has shown to aggravate several cancer hallmarks, including increased cellular motility, invasiveness and angiogenesis and is, thereby providing an explanation for the observed impact under clinical conditions.\nIn order to investigate whether GIRK1 also act cancerogenic on benign MCF10A mammary epithelial cells, overexpressing GIRK1 have been engineered. Validation and characterization of mammary epithelial cell (MEC) lines overexpressing GIRK1, examined by qPCR and Western Blot analysis. Membrane resting potentials (RPs) were compared to controls. Transcriptome analysis reverted that the overexpression of GIRK1 in benign MECs affects expression levels of about 1900 genes involved in several cellular pro-tumorigenic pathways. Visualization and Integrated Discovery (DAVID) Pathway Analysis revealed that many of these transcripts are regulated towards specific cellular functions and pro-tumorigenic actions. Heat maps display the quantitative effect on selected gene clusters and underscore the amount of cellular regulation exerted by GIRK1 overexpression. Upon GIRK1 overexpression, motility and cellular velocities of the benign MCF10A control lines were substantially increased and associated with transcriptional changes of genes that are linked to motility at the cellular level.\nThe most prominently regulated functional cluster is upregulated and comprises downstream elements which are triggered by the cytokine interferon-γ (IF-γ). In the present study, overexpression of GIRK1 in MCF10A cell line triggered several downstream IF-γ signaling elements known to act pro-tumorigenic in MEC lines. We observe profound changes in cellular phenotype upon GIRK1 overexpression in benign MCF10A. In this examination, it was found that the overexpression of only one single K+-channel subunit, which is of high importance to a cell and, can shift the vital parameters of a benign MEC-line towards cancer hallmarks. Research has shown that disturbance with potassium\n13\nchannels potentially offers a new therapeutic window for the subtype of estrogen receptor positive (ER+) breast cancer treatment.
