            "title": "Die Qualität von Reanimationsmaßnahmen am Notarztstützpunkt des LKH-Universitätsklinikums Graz - eine retrospektive, computergestützte Betrachtung von Gerätedaten",
            "abstract": "Der plötzliche Herzkreislaufstillstand ist der zeitdringlichste medizinischer Notfall, da nach wenigen Minuten ohne Reanimationsmaßnahmen irreversible Zellschäden auftreten. In Europa ist der plötzliche Herzkreislaufstillstand die dritt häufigste Todesursache bei Erwachsenen. Die Inzidenz des plötzlichen Herzkreislaufstillstandes außerhalb des Krankenhauses liegt bei 89 pro 100.000 Einwohner*innen. Die Qualität der durchgeführten Reanimationsmaßnahmen, insbesondere der Thoraxkompressionen (Teil der Cardiopulmonale Reanimation – CPR), ist maßgeblich für ein günstiges Ergebnis. Die Qualität kann anhand Daten, die während der CPR vom Defibrillator aufgezeichnet werden, beurteilt werden. Am Standort der Notarzteinsatzfahrzeuge (NEF) des Universitätsklinikum Graz werden diese Daten automatisch aufgezeichnet. Bisher ist jedoch eine routinemäßige Auswertung dieser Daten und deren Verwendung in Studien nicht möglich. Daher stellt sich die Herausforderung, diese Daten benutzungsfreundlich zugänglich zu machen, um sie für das Qualitätsmanagement und Forschungsarbeiten nutzbar zu machen. Ziel dieser Arbeit ist die Implementierung einer graphischen Oberfläche zur Steuerung einer Datenbank, anhand welcher die Speicherung, Auswertung und Vorselektion von präklinischen Reanimationsdaten möglich ist. Dazu sollen Algorithmen implementiert werden, die Thoraxkompressionen anhand der Thoraximpedanz und Beatmungen mittels Kapnographiesignal erkennen.\r\nEine SQL-Datenbank mit 522 Reanimationsfällen wurde erfolgreich aufgesetzt und kann über eine grafische Benutzeroberfläche gesteuert werden. Darüber können Fälle nach dem Vorhandensein von bestimmten Signalen gefiltert werden und für einzelne Fälle können die Signale grafisch dargestellt werden. Von den 522 Fällen wurden nur 425 in die Datenbank importiert und 97 automatisch aussortiert, da sie keine oder keine relevanten Impedanzsignale enthielten. Aus diesen 425 Fällen wurden Kenngrößen für die CPR-Qualität bestimmt. Die mittlere Thoraxkompressionsrate der Fälle lag bei 116,3 ± 9,1 Kompressionen pro Minute und damit im Zielbereich zwischen 90–120. In 31% der Fälle wurde eine zu hohe Rate festgestellt. Der Median für die Thoraxkompressionen pro Minute bezogen auf die gesamte Reanimationsdauer lag bei 99,2 ([89,4 – 106,7]) und der Anteil an Minuten ohne adäquater mittlerer Thoraxkompressionsrate bei 29,2 ([6,2; 60,4]) %. Die mittlere Beatmungsfrequenz aller Fälle mit Kapnographiesignal (150 Fälle) lag bei 13,5 ± 3,6 Beatmungen pro Minute und damit innerhalb des Zielbereichs von maximal 15 Beatmungen. Jedoch wurde in 34% der Fälle eine Beatmung mit zu hoher Beatmungsfrequenz durchgeführt. Der Anteil an Minuten mit Hyperventilation betrug im Median 24,0 [5,6 - 48,6] % und der Anteil an Minuten ohne Beatmung lag im Median bei 6,2 [2,0 - 16,6] %. Die kumulierte Pausenzeit betrug im Median 117,7 [53,0; 280,7] Sekunden und der Anteil der Pausenzeit an der gesamten Reanimationsdauer lag im Median bei 13,1 [8,2 - 21,9] %.\r\nDiese Arbeit zeigt, dass sich eine systematische und automatisierte Auswertung von Defibrillatoraufzeichnungen zur Beurteilung der Reanimationsqualität eignet. Die Bestimmung der Kenngrößen ergab plausible Ergebnisse und deckt sich mit Ergebnissen in Vorarbeiten an der Medizinischen Universität Graz, die noch händisch an signifikant kleineren Datensets durchgeführt wurden.",

            "title": "Interprofessionelles Dysphagie-Management bei Patient*innen mit Insult – Ein Scoping Review",
            "abstract": "Hintergrund: Weltweit ereignen sich jährlich über 12,2 Millionen Schlaganfälle. Die Dysphagie ist die häufigste Folgeerscheinung eines Insults und geht mit einer erhöhten Mortalität und Morbidität einher. Im klinischen Setting arbeiten viele Gesundheitsprofessionen zusammen, deren Aufgabenbereiche sich sowohl überschneiden als auch eng miteinander verknüpft sein können. Aus diesem Grund ist eine frühzeitige Beurteilung, Behandlung und Betreuung einer insultbedingten Dysphagie im interprofessionellen Team notwendig.\r\n\r\nZiel: Ziel ist es aufzuzeigen, welche Maßnahmen im interprofessionellen Team bei Patient*innen mit insultbedingter Dysphagie gesetzt werden können und wie nachfolgend eine Verbesserung der interprofessionellen Zusammenarbeit erzielt werden kann.\r\n\r\nMethode: Es wurde ein Scoping Review durchgeführt. Eine systematische Literaturrecherche fand zwischen April und Juni 2023 in den internationalen Datenbanken CINAHL, PubMed, Cochrane Library, Embase via Ovid sowie in der Suchmaschine Google Scholar und mittels Handsuche in den Referenzlisten der eingeschlossenen Studien statt. Vorab wurden Ein- und Ausschlusskriterien fest-gelegt, die anhand der Merkmale Population, Konzept, Kontext und Art der Literatur definiert wurden. Im Anschluss erfolgte eine tabellarische Datenextraktion und narrative Synthese der Ergebnisse.\r\n\r\nErgebnisse: 14 Studien wurden zur Beantwortung der Forschungsfragen aus der internationalen Literatur erfasst. Die Anwendung von validen und reliablen Screening-Tools, kompensatorischen Techniken wie Schluck-, Zungen- und Atem-muskeltrainings, professioneller Mundpflege, konsistenz-adaptierter Nahrung und Flüssigkeit, adäquater Positionierung bei der Mahlzeiteneinnahme sowie psychologische Unterstützung konnten unter anderem zu einer signifikanten Verringerung in der Häufigkeit von Pneumonien, einer Reduzierung der Krankenhausverweildauer und einer Verbesserung der Schluckfunktion der Patient*innen führen. Identifiziert wurden weitere organisatorische Maßnahmen, wie klar definierte Kommunikationswege, interprofessionelle Behandlungsprotokolle und regelmäßige interprofessionelle Besprechungen, welche die interprofessionelle Zusammenarbeit bei Patient*innen mit insultbedingter Dysphagie verbessern können.\r\n\r\nSchlussfolgerung: Es konnten nur wenige Studien identifiziert werden, welche sich mit dem interprofessionellen Dysphagie-Management bei Patient*innen mit Insult beschäftigen. Da viele bestehende Screening-Tools zur Risikoerfassung einer Dysphagie bisher nicht auf ihre Validität und Reliabilität geprüft wurden, ist eine Überprüfung der psychometrischen Eigenschaften notwendig. Zudem besteht weiterer Forschungsbedarf, um umfassendere strukturelle und organisatorische Maßnahmen sowie deren Effektivität im Rahmen des interprofessionellen Dysphagie-Managements bei Patient*innen mit Insult zu untersuchen.",

            "title": "Modeling NF1 loss-of-function mutation in hormone receptor-positive breast cancer cells",
            "abstract": "Metastatic breast cancer remains an incurable disease and accounts for the majority of cancer-related deaths in women worldwide. The most common subtype are hormone- receptor-positive tumors, characterized by a dependence on the female sex hormones estrogen and progesterone. Implicitly, endocrine therapy is the standard of care complemented by CDK4/6 inhibitors that induce a cell cycle arrest and impede tumor cells from growing. However, therapy resistance and consecutive tumor progression remain significant clinical problems and therefore, the currently most promising research projects are translational approaches to understand therapy resistance mechanisms better and find novel targets to improve therapy options steadily.\r\nTo demonstrate the current practice of translational research, a liquid biopsy-based detection of an NF1 mutation in a stage four breast cancer patient with progressive disease treated at our university hospital led to the start of an in vitro project. Therefore, we introduced an NF1 mutation with CRISPR/Cas 9 genome editing in hormone-receptor-positive MCF-7 breast cancer cells and evaluated the response of the mutated cells to standard first-line CDK4/6 inhibitors (abemaciclib, palbociclib, and ribociclib) as well as the endocrine therapy agent fulvestrant. The main focus of our experiments was directed at cell cycle alterations. In order to put our findings into perspective, we also generated a retinoblastoma protein 1 (RB1)- deficient MCF-7 cell line since RB1-loss-of function is linked to primary CDK4/6 inhibitor resistance and, therefore, the ideal positive control in our therapy resistance study.\r\nOur NF1-deficient cells grew significantly faster and showed a decreased sensitivity to CDK 4/6 inhibitors in our drug treatment experiments seen in cell-cycle alterations and a decreased extent of the CDK4/6 induced senescent-like phenotype. Interestingly, we also observed that our NF1 mutated cells preserved their highest sensitivity to the CDK4/6 inhibitor ribociclib. Expectedly, RB1 loss-of-function resulted in a cross-resistance to all three CDK4/6 inhibitors, rapid growth, and an epithelial to mesenchymal transition of their cellular state.\r\nThe model developed in this thesis allows us to study the role of genetic alterations detected by dynamic liquid biopsy findings in vitro to better understand and eventually overcome therapy resistance mechanisms in the future.",

            "title": "Comparison of cardiac MRI and endomyocardial biopsy in the diagnosis of myocarditis",
            "abstract": "Introduction\r\n\r\nCardiac MRI and endomyocardial biopsy represent the current noninvasive and invasive gold standard of myocarditis-diagnosis. We aimed to investigate the diagnostic concordance of endomyocardial biopsy and cardiac MRI in the diagnosis of myocarditis. In the sub analysis, we investigated whether performing endomyocardial biopsy prior to cardiac MRI leads to an increased number of false-positive myocarditis diagnoses.\r\n\r\nMethods\r\n\r\nWe performed a retrospective data analysis. One hundred sixty-six patients were included in our main analysis who received biopsy and MRI in a time frame of 30 days between 2008 and 2020 at our university clinic. For the sub analysis we included 147 patients who received both investigations within 14 days. The patients were divided into the groups 'biopsy before MRI' and 'MRI before biopsy'. We compared the groups regarding the occurrence of Lake Louise myocarditis criteria and MRI myocarditis diagnoses and calculated the positive predictive value (PPV) and negative predictive value (NPV) for the MRI with biopsy-confirmed myocarditis as a reference.\r\n\r\nResults\r\n\r\nIn our main analysis 119 (72%) patients were diagnosed with myocarditis in either biopsy or MRI. In the MRI reports, 61 (37%) patients were diagnosed with myocarditis, compared to 101 (61%) inflammatory cardiomyopathy diagnoses in EMB reports. In 90 (54%) patients, a concordant diagnosis was made in biopsy and MRI with regard to the question of whether myocarditis was present or absent. Comparing groups in our sub analysis, frequency of detected non-ischemic LGE (60.0% versus 73.9%, p=0.078) and edema (16.4% vs. 24.4%, p=0.249) was not significantly different. In group ‘biopsy before MRI’ (n=55), 92% of MRI-suspected myocarditis cases were concordantly diagnosed with myocarditis in their biopsy reports with a NPV and PPV of 42.9% and 92.3%. In contrast, in group ‘MRI before biopsy’ (n=92) only 60% of MRI-suspected myocarditis cases were concordantly diagnosed in their biopsy reports with a NPV and PPV of 42.3% and 60.0%.\r\n\r\nConclusio\r\n\r\nWe found moderate concordance of MRI and endomyocardial biopsy in the diagnosis of myocarditis. Performing endomyocardial biopsy prior to cardiac MRI did neither result in increased incidences of nonischemic late gadolinium enhancement or myocardial edema, nor did it result in an increase of false-positive diagnoses of myocarditis in MRI.",

            "title": "In-vitro Untersuchung der Zytotoxizität von bioresorbierbaren Mg-Legierungen anhand der Stoffwechselaktivität kultivierter MC3T3-Osteoblasten",
            "abstract": "Traditionelle metallische Implantate konnten sich über Jahrzehnte als bewährte Mittel zur Frakturfixation beweisen. Heutzutage steigt allerdings die Nachfrage nach bioresorbierbaren Alternativen auf Magnesium-Basis um Kosten, post-operative Risiken und erneute Eingriffe zur Entfernung zu reduzieren beziehungsweise zu verhindern.\r\nZielsetzung:\r\nZiel dieser Arbeit ist es festzustellen, ob die modernen bioresorbierbaren Mg-Legierungen ZX00 und WE43 eine adäquaten Biokompatibilität für den klinischen Alltag vorweisen und ob deren unterschiedlich rasches Degradierungsverhalten das umliegende Gewebe negativ beeinflusst.\r\nMaterial und Methoden:\r\nEs handelt sich um einen in vitro Versuchsaufbau bei dem die Mg-Legierungen ZX00 und WE43 reinem Titan (Grad 2) gegenübergestellt wurden. Als Zellbasis wurden MC3T3-E1 Zellen verwendet. Der Versuchsaufbau basiert auf einem CellTiter 96® AQueous Non-Radioactive Cell Proliferation Assay und es wurden pro Material vier Versuchsdurchgänge in jeweils 5 Verdünnungen durchgeführt. Abschließend wurde die Zellaktivität photometrisch vermessen und mittels Fluoreszenzmikroskop geprüft und fotografiert.\r\nErgebnisse: \r\nNach statistischer Auswertung zeigte sich, dass in purer Lösung eine klare Abnahme (ZX00: -88,39%; WE43: -88,32%) der Zellaktivität und somit der vitalen Zellen feststellbar ist. In Verdünnung hingegen zeigten beide untersuchten Mg-Legierungen allerdings positive Ergebnisse. Mit Ausnahme von WE43 in 1:2 Verdünnung (minimale Abnahme von -2,42%) konnte sogar ein genereller Anstieg der Zellaktivität und somit der vitalen Zellen festgestellt werden.\r\nSchlussfolgerungen:\r\nDiese Studie kommt zu dem Schluss, dass ZX00 und WE43 eine geeignete Biokompatibilität für die klinische Anwendung besitzen. Ausschließlich im Verdünnungsverhältnis 300 µl Materiallösung zu 100 µl Zellmedium konnte eine signifikante Reduktion der vitalen Zellen festgestellt werden. Alle anderen betrachteten Verdünnungen zeigten hingegen mit TiGr2 vergleichbare positive Ergebnisse. Weitere Abklärung bedarf es vor allem im Rahmen von Co-Implantationen mit anderen Biomaterialien aufgrund der dabei entstehenden galvanischen Korrosion, sowie im Bereich der Materialherstellung bezüglich Oberflächenmodifikationen zur Reduktion der Resorptionsrate und somit der Verringerung der H2-Gasakkumulation im periimplantären Gewebe.",

            "title": "Eignung der perioperativen Gabe von Bromelain für die Minderung postoperativer Beschwerden im Sinne von Schwellung und Schmerzen nach der Entfernung retinierter Weisheitszähne – eine Literaturanalyse",
            "abstract": "Hintergrund:\r\nDie operative Entfernung retinierter Weisheitszähne stellt einen Standardeingriff im zahnmedizinischen Fachbereich der Oralchirurgie dar. In Verbindung damit werden häufig postoperative Beschwerden, wie Schmerzen, Schwellung und Inflammation genannt. Gängige Arzneimittel, die im Rahmen der Weisheitszahnentfernung verabreicht werden, sind NSAR, Kortikosteroide und Antibiotika. Die genannten Arzneimittel bergen jedoch das Risiko signifikanter Nebenwirkungen. Ebenso steigen aufgrund prophylaktischer Verabreichung von Antibiotika auch die Zahlen der Antibiotikaresistenzen. Aufgrund dessen konnte ein wachsendes Interesse an pflanzlichen alternativen, wie proteolytische Enzyme, verzeichnet werden. Bromelain, ein Extrakt aus der Ananas, soll in diesem Kontext die gewünschten antiödematösen, antiinflammatorischen und analgetischen Eigenschaften liefern.\r\nIn der aktuellen Literatur herrschen noch Unstimmigkeiten, inwiefern Bromelain die die postoperativen Beschwerden nach der operativen Entfernung retinierter Weisheitszähne lindern kann. \r\n\r\nMaterial und Methode:\r\nUm die Frage der Eignung von Bromelain für die Reduktion postoperativer Beschwerden, wie Schmerzen und Schwellung, zu ergründen, wurde im Rahmen dieser Diplomarbeit eine systematische Literaturanalyse durchgeführt. Dafür wurden verschiedene Datenbanken und Fachbücher für geeignete Referenzen gesichtet. Im Zentrum standen dabei die Suchbegriffe ‚bromelain‘ und ‚third molar‘ inklusive Varianten. \r\n\r\nErgebnisse:\r\nNach anschließender Selektion durch die Durchsicht der Titel, Abstracts und anschließend der Volltexte konnten neun Artikel herausgefiltert werden.\r\nBei diesen Artikeln handelt es sich um verschiedene Studien, die im Rahmen der retinierten Weisheitszahnentfernung die Wirkung von Bromelain untersuchten. Dabei verglichen manche Studien Bromelain mit gängigen, in diesem Kontext verwendeten, Medikamente, andere verwendeten Placebos als Kontrolle. Für die Quantifizierung der Wirkung wurden Parameter, wie Schmerz, Schwellung und Trismus durch verschiedene Methoden gemessen. Aus dem Vergleich der selektierten Studien konnte die Erkenntnis gewonnen werden, dass die untersuchten Studien sehr heterogene Ergebnisse lieferten und die Meinungen zur Wirkung von Bromelain nicht einheitlich sind. Nicht in allen Studien und nicht hinsichtlich aller Parameter konnten positive Effekte von Bromelain festgestellt werden.\r\n\r\nKonklusion:\r\nDie vorliegende Arbeit zeigt, dass Bromelain bei der Weisheitszahnentfernung in der richtigen Dosis und dem richtigen Einnahmeschema positive Auswirkungen auf das postoperative Wohlbefinden von Patient*innen haben kann. Für signifikante klinische Ergebnisse sind dennoch weitere Studien sinnvoll. Ein besonderes Augenmerk sollte dabei auf das Studiendesign gelegt werden. Studien mit größeren Fallzahlen und prospektivem Design, welche im Crossover Verfahren durchgeführt werden, scheinen geeignet, um eine Kausalität feststellen zu können.",

            "title": "Kardiovaskuläre Reaktivität- Eine Pilotstudie im klinischen Umfeld",
            "abstract": "Hintergrund: Stürze in der älteren Bevölkerung können aufgrund der schwerwiegenden Folgen, wie beispielsweise Frakturen zum Verlust der Selbstständigkeit und weiteren Folgeerkrankungen führen. Die Risikofaktoren für Stürze zu identifizieren und positiv zu beeinflussen wäre dabei ein relevanter Schritt in der Prävention. Um einen Zusammenhang zwischen den Veränderungen des alternden Herzkreislaufsystems, welches weniger effizient auf veränderte Bedingungen oder Belastungen reagieren kann und Stürzen herzzustellen, werden die Herzfrequenz und Parameter der Herzratenvariabilität aufgezeichnet. Zum Vergleich wird eine Kontrollgruppe mit negative Sturzanamnese herangezogen. \r\n\r\nMethoden: In diese Studie wurden 21 Patient*innen, die sich stationär auf Abteilungen für Akutgeriatrie Remobilisation (AG/R) der geriatrischen Gesundheitszentren der Stadt Graz (GGZ) befanden eingeschlossen und zwei Gruppen zugeordnet (10 mit negativer Sturzanamnese in den letzten 12 Monaten und 11 mit Schenkelhalsfraktur und positiver Sturzanamnese [SHF]). Neben den Fragebögen Barthel-Index und BDI II wurde kardiovaskuläre Parameter mittels EKG unter Ruhebedingungen und unter den milden Stressoren Handkraft und Stroop-Test erhoben. Neben der Herzfrequenz wurden die HRV- Parameter für jede einzelne Phase ausgewertet. Das Paradigma wurde zu Beginn der des stationären Aufenthalts und kurz vor der Entlassung durchgeführt. Neben dem Vergleich genannter Messzeitpunkte erfolgte ein Vergleich zwischen den Gruppen.\r\n\r\nErgebnisse: Es ergaben sich Hinweise, dass die Patient*innen der SHF- Gruppe aufgrund von Frailty- assoziierten Parametern (Handkraft, Barthel- Index, Herzfrequenzparameter) ein höheres Sturzrisiko aufweisen als die Kontrollgruppe. Hinsichtlich der kardiovaskulären Reaktivität auf die milden Stressoren Handkraft und Stroop- Test zeigten sich ebenfalls keine Gruppenunterschiede. Positive Veränderungen der Parameter nach etwa drei Wochen Remobilisation konnten entgegen den Erwartungen nicht nachgewiesen werden. \r\n\r\nKonklusion: Patient*innen mit positiver Sturzanamnese haben ein höheres Sturzrisiko als Patient*innen mit negativer Sturzanamnese und verfügen über ausgeprägtere Risikofaktoren hinsichtlich Frailty und Sarkopenie. Dem entsprechend kommt der Identifikation und Reduktion von Risikofaktoren für Stürze eine große Bedeutung zu. Daher kann diese Studie Einblicke in die kardiovaskulären Parameter geriatrischer Patient*innen und der Identifikation von Risikofaktoren für Stürze geben.",
