Diese Strukturen können für jeden Patienten einzeln gefertigt werden, um eine präzise Gestaltung zu ermöglichen und eine schnelle und effektive Knochenregeneration zu unterstützen. Ein weiterer Vorteil ist die Verwendung von resorbierbaren Materialien, um weitere Entfernungsoperationen zu vermeiden und das Risiko für Patienten zu minimieren. \r\nDiese Arbeit präsentiert den aktuellen Forschungsstand zu personalisierten 3D-gedruckten Gerüsten und beschreibt einen experimentellen Aufbau, um deren Eignung für die Knochenregeneration zu testen.\r\nDas Ziel bestand darin, eine Designstrategie für ein geeignetes Gerüst zu entwickeln und diese Strategie dann in einem In-vitro-Experiment zu testen.\r\nDie entwickelten Gerüste haben eine Dicke von 500 µm, 750 µm bzw. 1000 µm und runde Poren mit einer Größe von 600 µm. Für die Tests wurde der Standarddefekt in einem\r\nSchweinekieferknochen gefräst und anschließend gescannt, um das Erstellen einer DVT-Datei und die Reproduktion des Modellkieferknochens mithilfe digitaler Abdrücke zu ermöglichen.\r\nAnschließend wurden sie in Polycaprolacton (PCL) nachgedruckt. Es wurden Schrauben, individuelle Meshes, 18 Kieferknochenproben aus PCL mit Standarddefekten und\r\nKnochenersatzmaterial verwendet. Bei den Proben wurden jeweils 0,25 g Bio-Oss® verwendet. Das Bio-Oss® wurde mit destilliertem Wasser gemischt, sodass das Knochenersatzmaterial eine leicht klebrige Konsistenz aufwies.\r\nDie Meshes wurden auf die Standarddefekte platziert. Die Schrauben wurden mit einem Schraubendreher in die Löcher eingedreht, bis sie bündig mit der Oberfläche waren. Der\r\nVorgang wurde wiederholt. Es wurden Messungen des Abstands zwischen dem Scaffold und dem Kieferknochen durchgeführt.\r\nVorläufige Ergebnisse deuten auf eine Genauigkeit der Passform zur Defektseite hin. Die Ergebnisse wurden in Tabellenform dargestellt und sie wurden im Hinblick auf ihre Eignung interpretiert.\r\nIm Falle positiver Ergebnisse könnten die präklinischen Studien den Weg für vollständig resorbierbare, biokompatible und osteokonduktive geführte Knochenregeneration ebnen. Due to the rarity of these tumors, guidelines on the optimal clinical management of these molecularly defined entities are lacking. \r\nMethods: The present work aims at providing an overview of diagnostic and therapeutic strategies based on a PubMed research and case reports of 7 patients with rare molecularly defined soft tissue sarcomas treated at the Division of Pediatric Hematology and Oncology, Medical University of Graz (2010-2023). \r\nResults: 53% (49/93) of patients with IFS harbored gene fusions other than the canonical ETV6-NTRK3 (NTRK1-3, BRAF, MET, ALK, RET). Surgical resection only (complete or incomplete) was curative in 79% (27/34) of patients with localized IFS, while relapse occurred in 47% (7/15) of patients with incomplete resection. Of 30 patients with IFS receiving targeted therapy, 15 (50%) achieved CR, none of them died. Of 50 patients with DSRCT, 64% (30/47 with available follow-up) died despite aggressive multimodal treatment (median OS: 1.5 years). Similarly, prognosis is unfavorable in patients with CRS (67% [16/24] died) and CCSS (60% [12/20] died). Of 33 patients with BRS who commonly received multimodal treatment, 81% (25/31) were alive at last follow-up, including 61% in CR. In 24 patients with NTRK-rearranged spindle cell neoplasms, 100% (24/24) were alive at last follow-up (median: 2 years), mainly following primary or secondary R0-resection. Despite 88% of patients with AFH receiving surgery only (n=22), disease was reported extensive and letal in some patients. \r\nConclusion: In NTRK-rearranged spindle cell neoplasms and AFH, treatment is based on surgery only. In BRS, multimodal treatment concepts are advised. CRS, DSRCT and CCSS show an unfavorable prognosis. Schätzung aufgrund einer Analyse einer 11-jährigen Geburtskohorte in Graz", "abstract": "Einleitung: Erfahrung in der Betreuung von Frauen mit weiblicher Genitalverstümmelung (female genital mutilation/cutting, FGM/C) ist in Österreich begrenzt. Gerade diese Frauen benötigen jedoch eine spezialisierte gynäkologische und geburtshilfliche Betreuung. In der Steiermark gibt es derzeit weder eine offizielle Beratungsstelle noch speziell geschultes medizinisches Personal, das auf die spezifischen Bedürfnisse von Frauen nach FGM/C eingeht. Ziel dieser Studie war es, die Anzahl an Geburten durch von FGM/C betroffene Frauen an der Grazer Frauenklinik zu schätzen, um den potenziellen Bedarf an geburtshilflicher Versorgung von Frauen nach FGM/C in unserer Region zu ermitteln. \r\nMethodik: Wir untersuchten alle Geburten am LKH-Universitätsklinikum Graz von 2010-2020 im Hinblick auf den Geburtsort und/oder die Nationalität der Mutter, um Entbindungen von Frauen aus einem Land mit bekannter FGM/C-Prävalenz herauszufiltern. Wir überprüften, ob FGM/C dokumentiert wurde und verglichen die Ergebnisse mit der erwarteten FGM/C-Rate basierend auf der Globalen Datenbank von UNICEF (1). Weiters erhoben wir Daten zum Geburtsverlauf und peripartalen Komplikationen. \r\nErgebnisse: Im Untersuchungszeitraum gab es insgesamt 35.628 Entbindungen, von denen 856 (2,41%) Geburten auf Frauen aus einem Land mit bekannter FGM/C-Prävalenz entfielen. Am häufigsten stammten die von FGM betroffenen Frauen aus unserem Kollektiv aus Ägypten, Nigeria, Irak, Ghana und Somalia. Wir fanden lediglich 22/856 (2,6%) dokumentierte FGM/C-Fälle bezogen auf die Entbindungen und 17/539 (3,2%) dokumentierte Fälle bezogen auf die Frauen. Die geschätzte FGM/C-Rate betrug 318/856 (37,1%). Der Entbindungsmodus bei Frauen aus unserem Studienkollektiv war mit 55% eine Spontangeburt, mit 4,9% eine assistierte vaginale Geburt und mit 40,1% ein Kaiserschnitt. Mehr als 90% der Frauen aus Ländern mit FGM/C-Prävalenz waren bereits vor der Entbindung an der Grazer Frauenklinik vorstellig. \r\nDiskussion: Die große Diskrepanz zwischen der dokumentierten und der erwarteten FGM/C-Rate (2,6% vs. 37%) in unserem Kollektiv deutet darauf hin, dass die meisten Fälle von FGM/C bei Frauen, die in unserer Region entbinden, unerkannt bleiben.
