Dieses Referenzkollektiv wurde deswegen gewählt, da ein gewisses Maß an körperlicher Betätigung die Norm darstellen sollte, um damit Wohlstandserkrankungen frühzeitig entgegenzuwirken.\r\nMaterial und Methodik:\r\n107 sportlich aktive Probanden im Alter von 18 bis 80 Jahren (47 Frauen und 60 Männer) wurden mittels der Referenzmessung Ganzkörper¿Dual-Energy X-Ray Absorptiometrie (DXA) und den gemessenen Widerstandswerten (5 kHz und 400 kHz) der WB-BIS und S-BIS (ImpediMed SFB7) verglichen. 80 Probanden (85,6%) wurden neuerlich nach ein bis vier Wochen gemessen.\r\nErgebnisse:\r\nDie Reproduzierbarkeit der einzelnen Körperabschnitte der segmentalen BIS ergab eine Korrelation nach Pearson zwischen r=0,74 und r=0,97. Die Vorhersage der Muskelmasse durch segmentale BIS erzielte eine Verbesserung des Vertrauensbereiches (± 1,96 SD ) von ± 7,82kg (Anthropometrie) und von ± 4,67kg (WB-BIS) auf ± 3,19kg. Die entsprechenden Zahlen für den Vertrauensbereich der Vorhersage der Fettmasse waren ± 7,96kg; ± 4,57kg und ± 3,31kg.\r\nSchlussfolgerung:\r\nDie Verwendung der segmentalen Impedanz bei zwei Frequenzen ist eine vielversprechende neue Methode, um MM und FM einfach und sehr genau zu messen. Über die Behandlung der zu Grunde liegenden Infektion und allgemeine Maßnahmen hinaus existiert keine spezifische Therapie. Neuere Therapieansätze, wie Hydrokortison, intensivierte Insulintherapie, aktiviertes Protein C oder TLR-4 Antagonisten, waren in klinischen Studien nicht überzeugend und werden in den aktuellen Leitlinien nicht oder nur bedingt empfohlen.\r\nDie milde Hypothermie (MH) ist Leitlinientherapie zur Neuroprotektion nach Herzstillstand. Kardiovaskulär steigert MH den peripheren Widerstand, ist positiv inotrop und vermindert den Gesamtkörpersauerstoffverbrauch. MH könnte somit der septischen Kardiomyopathie und dem septischen Schock entgegenwirken.\r\n\r\nMethoden\r\n13 Hausschweine (65±2kgKG) wurden mit Midazolam, Fentanyl, Pancuronium und Sevofluran anästhesiert und bei geschlossenem Thorax mit Kathetern instrumentiert: Swan-Ganz-Katheter, linksventrikulärer Druck-Volumenkatheter, Valvuloplastiekatheter in die Aorta descendes und ein intravenöser Kühlkatheter. \r\nDurch Infusion von LPS (Lipopolysaccharid) mit einer Rate von 0,5µg/kgKG/h für eine Stunde und anschließend einer Rate von 1.0µg/kgKG/h für weitere 3 Stunden wurde eine Endotoxinämie als Modell der Sepsis induziert. Mit Beginn der LPS Infusion wurden die Tiere entweder einer Normothermiegruppe (NT, 38°C) oder der Hypothermie-Gruppe (MH, 33°C) zugeordnet. Der Beobachtungszeitraum erstreckte sich ab der LPS Infusion über acht Stunden.\r\n\r\nErgebnisse\r\nNach 8 Stunden war der Cardiac Output in der NT-Gruppe höher als in der MH-Gruppe (MH 4.5±0.5l/min vs NT 6.6±0.4l/min). Der periphere Widerstand (NT 6.6±0.4mmHg/l/min vs MH 11.1±1.0mmHg/l/min) und die gemischtvenöse Sauerstoffsättigung (NT 54±3% vs MH 77±6%) waren in MH vs NT höher. Die arterielle Sauerstoffsättigung (NT 92±3% vs MH 100±0%) und der Oxygenierungsindex (paO2/FiO2; NT 94±12mmHg vs MH 359±31mmHg) waren unter MH ebenfalls höher. Die linksventrikuläre Kontraktilität war zwischen den Gruppen nicht verschieden, dies jedoch unter signifikant erhöhter sympathischer Aktivierung ausschließlich in NT. In isolierten Herzmuskelstreifen aus MH vs NT zeigte sich ein deutlicherer Anstieg der entwickelten Kraft bei Steigerung der Stimulationsfrequenz und während Stimulation mit Isoproterenol. Die Plasmakonzentration von TNFalpha war in MH gegenüber NT erhöht. Die Konzentrationen anderer Zytokine (IL-6, IL-8 und IL-10) waren nicht signifikant unterschiedlich.\r\n\r\nZusammenfassung:\r\nDie Induktion der milden Hypothermie verbessert Gefäßtonus, Lungenfunktion und zentrale Sauerstoffsättigung, verhindert die sympathische Aktivierung und verbessert die Myokardfunktion. Die dabei erhöhten Spiegel von TNFalpha deuten auf einen Effekt der MH distal der Zytokinkaskade hin. Aufgrund der Beziehung zur Wachstumsfuge, die mit 80% für das Längenwachstum des Oberarms verantwortlich ist, hat die proximale Humerusfraktur ein sehr großes Remodelingpotential. Die Behandlung erfolgt je nach Alter, Dislokationsgrad und der Begleitverletzungen entweder konservativ oder operativ. \r\nZiel dieser Studie war es, das langfristige Remodelingpotential nach operativ behandelter proximaler Humerusfraktur im Wachstumsalter sowie langfristige funktionelle Ergebnis dieser Frakturen zu untersuchen. In diese Studie wurden insgesamt 31 Patienten (18 m, 13 w) eingeschlossen, welche sich zwischen 2004 ¿ 2010 eine proximale Humerusfraktur zuzogen und an der Univ. Klinik für Kinder- und Jugendchirurgie Graz operativ behandelt wurden. Das durchschnittliche Alter zum Unfallzeitpunkt lag bei 11,3 (range: 3,1 ¿ 15,8) Jahren Alle Patienten wurden schriftlich zu einer klinischen und radiologischen Nachuntersuchung einbestellt. \r\n15 Patienten wurden mit einem elastisch-stabilen intramedullärem Nagel (ESIN) und 16 Patienten mit einer Bohrdrahtosteosynthese versorgt. Die durchschnittliche Zeit zwischen dem Unfall und der Nachuntersuchung betrug 5,8 (range: 1,1 ¿ 12,5) Jahre. Alle Patienten wurden klinisch mithilfe des DASH-Scores und des Constant-Scores validiert und die präoperativen Röntgenbilder wurden mit den Bildern der Nachuntersuchung verglichen. Es zeigte sich, dass sich alle Frakturen radiologisch sehr gut remodelliert haben. Klinisch schnitten die mit ESIN behandelten Patienten im DASH-Score im Arbeits- sowie im Sport und Musikmodul besser ab als die Patienten, die mit einer Bohrdrahtosteosynthese versorgt wurden. Im Allgemeinen Teil schnitten die ESIN Patienten jedoch schlechter ab. Im Mittel betrug der Wert im DASH-Score bei den mit ESIN behandelten Patienten 1,9 (range: 0 - 17,5) und 1,4 (range: 0 ¿ 12,5) Punkte bei den Patienten, die mit einer Bohrdrahtosteosynthese versorgt wurden. Aufgrund der hepatischen Dysfunktion sind Patienten mit Leberzirrhose besonders gefährdet für Blutungen, Infektionen und Komplikationen während des Eingriffs. Zahlreiche Studien haben die Überlegenheit beziehungsweise Gleichwertigkeit des MELD-Score im Vergleich zum Child-Turcotte Pugh gezeigt. Das Ziel unserer Studie war die Evaluierung des perioperativen Risikos nach verschiedenen chirurgischen Eingriffen, potenzielle Risikofaktoren für erhöhte Mortalität zu identifizieren sowie den prädiktiven Wert für bestimmte Laborparameter, aufgeführt in den Leitlinien der ÖGARI, zu untersuchen. \r\nMethoden: In einer retrospektiven Datenanalyse wurden 132 Patienten mit Leberzirrhose evaluiert, bei denen verschiedene chirurgische Eingriffe im Zeitraum 2005-2010 am Universitätsklinikum Graz durchgeführt wurden. Potenzielle perioperative Risikofaktoren, sowie der MELD-Score und der Child-Turcotte Pugh wurden ausgewertet. Als primärer Endpunkt wurde die 30-Tage Mortalität festgelegt. \r\nErgebnisse: Die perioperative Gesamtmortalität betrug 8,3%. Patienten, die den Endpunkt erreichten, hatten höhere MELD-Scores (MELD: 8 vs. 11, p=0,001). In der multivariaten Analyse waren nur ein MELD ¿ 13 und die Schwere des Eingriffs, als unabhängige Faktoren mit der Mortalität assoziiert. Univariat waren weiterhin Child C, Notfalleingriffe, Patienten gelistet für Lebertransplantation sowie Patienten, die intraoperativ Transfusionen benötigten, Risikofaktoren für Mortalität. Die Laborparameter Hämoglobin, Thrombozyten, INR und Kreatinin waren ebenfalls mit der perioperativen Mortalität assoziiert. \r\nSchlussfolgerung: Unsere Ergebnisse zeigen, dass der MELD-Score sowie die Schwere eines Eingriffs als unabhängige Prädiktoren mit niedrigem Outcome korrelieren. Hormone sensitive lipase (HSL) regulates the hydrolysis of acylglycerols and cholesteryl esters (CE) in various cells and organs, including enterocytes of the small intestine. Adipose triglyceride lipase (ATGL) is the rate-limiting enzyme mediating triglyceride (TG) hydrolysis. Deficiency of ATGL results in TG accumulation in multiple tissues, underscoring the critical role of ATGL in maintaining lipid homeostasis. Furthermore, ATGL was shown to produce peroxisome proliferator-activated receptor alpha (PPAR¿) ligands.\r\nHerein we elucidated the role of these 2 lipases in the small intestine by generating mice with an intestinal lack of HSL or ATGL, respectively.\r\nIntestinal HSL knock out (HSLiKO) mice exhibit increased CE but unchanged TG and diglyceride concentration in the jejunum. Lipid absorption studies revealed that HSLiKO mice have accelerated cholesterol but unchanged TG absorption. Cholesterogenic genes were downregulated in the proximal intestine of HSLiKO mice and consequently less cholesterol biosynthesis was observed.\r\nWe found decreased TG hydrolase activity and increased intracellular TG content in intestinal ATGL knock out (ATGLiKO) small intestines. Intragastric administration of [³H]trioleate resulted in TG accumulation in the intestine, whereas absorption into the systemic circulation was unchanged. Intraperitoneally injected [³H]oleate accumulated within TG in ATGLiKO intestines, indicating that ATGL mobilizes fatty acids from the systemic circulation absorbed by the basolateral side from the blood. Downregulation of PPAR¿ target genes suggested modulation of cholesterol absorption by intestinal ATGL. Accordingly, we observed delayed cholesterol absorption.\r\nIn summary we showed that both enzymes play a role in intestinal lipid metabolism.
