Derzeit werden zur Messung der Wirksamkeit klinische Parameter und bildgebende Verfahren verwendet, die jedoch teuer und zeitaufwendig sind und die Patient*innen ionisierender Strahlung aussetzen. Deshalb entwickelt sich die ctDNA (zirkulierende Tumor-DNA) zu einem wichtigen Biomarker in der Präzisionsonkologie, da ctDNA die Tumorlast widerspiegeln kann. Im Speziellen stellt „Fragmentomics“ einen neuen vielversprechenden Forschungsbereich dar, der biologische Merkmale und Fragmentierungsmuster von zellfreier DNA (cfDNA) untersucht. In dieser Studie strebten wir an, basierend auf cfDNA Fragment-Endmotiven einen neuen Zugang zur Tumorüberwachung zu entwickeln.\r\n\r\nMethoden: Für die Studie wurden 343 Plasmaproben von gesunden Spender*innen und metastasierten Krebspatient*innen mit Hilfe von sWGS (shallow whole-genome sequencing) analysiert, wobei die Fragment-Endmotive extrahiert wurden. Im nächsten Schritt wurde ein Distanzmessungsmodell für die aussagekräftigsten Endmotive genutzt, um einen quantitativen Wert (sogenannten M2-Wert) zu bestimmen, mit dem die ctDNA Fraktion im Blut abgeschätzt werden kann. Der M2-Wert wurde mit Hilfe von ichorCNA und einer hochauflösenden Mutationsanalyse (QIAseq Targeted DNA Panel) validiert.\r\n\r\n\r\n\r\nResultate: Unsere Daten zeigten, dass Fragment-Endmotive informative Merkmale darstellen, um ctDNA-Fragmente von cfDNA, die von gesunden Zellen stammen, zu unterscheiden. Der Vergleich zwischen unterschiedlichen Assays zeigte, dass unser M2-Ansatz der empfindlichste Assay war und ctDNA in 88.6% (im Vergleich zu 68% mit QIAseq und 43% mit ichorCNA) der Plasmaproben von Nierenkrebspatient*innen nachweisen konnte.
