Der Goldstandart in der Therapie des fortgeschrittenen ZK ist die Radiochemotherapie (RCT). Ihr therapeutischer Effekt bei paraaortalen Lymphknotenmetastasen (PALK META) ist jedoch fraglich.\r\nAus diesem Grund haben wir eine retrospektive Analyse der Datenbank der Frauenklinik der Universität Graz (UFK Graz) über den Zeitraum von 1985-2008 durchgeführt. Hundertneunzehn Patientinnen mit einem ZK der FIGO-Stadien Ib bis IIIb, bei welchen eine systematische pelvine (PLA) und paraaortale Lymphadenektomie (PALA) vollzogen wurde, konnten eingeschlossen werden und in Bezug auf 5-Jahres Gesamtüberleben, rezidivfreies Überleben, Rezidivrate und Rezidivlokalisation hin untersucht werden. Das mediane Follow-up lag bei 7,9 Jahren. \r\n\r\nBei 81 Patientinnen (68%) konnten Lymphknotenmetastasen (LK META) nachgewiesen werden.\r\nVon den 119 Fällen waren 38 (32%) nodal negativ (N0), 2 (2%) hatten parametran positive LK, 57 (48%) waren pelvin positiv und 22 (18%) paraaortal positiv. Alle Patientinnen, bis auf eine, mit PALK META hatten auch Metastasen in den pelvinen LK. \r\nDie Rezidivrate betrug für N0 22,2%; für Patientinnen mit pelvinen Metastasen (PEL META) 39,7% und für PALK META 74,6%.\r\nDie Rezidive zeigten sich bei 31 Patientinnnen (26%) im Becken, bei 9 (7%) disseminiert in verschiedenen Organen, bei weiteren 9 (8%) in singulären Organen lokalisiert und 70 der 119 Patientinnen (59%) überlebten ohne Rezidive.\r\nUnter 22 Fällen mit PALK META traten alle 15 Rezidive innerhalb der ersten 4 Jahre post operationem (post OP) auf. Bei 34 Patientinnen die keine PALK META hatten, traten 3 Rezidive nach 5 Jahren post OP auf. Ein Rezidiv trat im 6. Jahr, 1 Rezidiv im 7. Jahr und ein Rezidiv im 14. Ihr Einsatz an immer älter werdenden und dadurch oft multimorbiden Patienten erfordert eine genaue Planung des Anästhesieverfahrens. Dafür sind Informationen über das Vor- und Begleiterkrankungsprofil der betroffenen Patienten von entscheidender Bedeutung. Zum Zeitpunkt der Literaturrecherche gab es keine wissenschaftliche Untersuchung, die auf Grundlage eines ausreichend großen Patientenkollektivs in der Lage war, eine Aussage über das Vor- und Begleiterkrankungsprofil von Patienten, welche sich einer neuroradiologischen Intervention unterziehen müssen, zu treffen.\r\n\r\nMaterial und Methoden: Es wurde eine retrospektive Analyse mit Datenmaterial im Kontext mit 900 anästhesiologisch begleiteten neuroradiologischen Interventionen durchgeführt. Neben demographischen Parametern wurden dabei indikationsbezogene, eingriffsbezogene, anästhesiebezogene und anamnesebezogene Parameter erhoben. \r\nSämtliche in die Untersuchung eingeschlossenen Parameter wurden in einer Microsoft®Access® - Datenbank gesammelt und einer deskriptiven statistischen Analyse unterzogen. \r\n\r\nErgebnisse: Anhand der im KIS dokumentierten Patientengeschichten sowie der periinterventionell erhobenen Patientendaten, konnten an einem Kollektiv von 900 anästhesiologisch begleiteten neuroradiologischen Interventionen, 111600 Einzeldaten in Bezug auf das Vor- und Begleiterkrankungsprofil der betroffenen Patienten erhoben werden. Es wurden als Hauptindikationen zerebrale Aneurysmen (n=264), zerebrale arteriovenöse Malformationen und zerebrale arteriovenöse Fisteln (n=162), zerebrale Stenosen (n=335), sowie zerebrale Thrombosen (n=34) in vier Diagnosegruppen zusammengefasst. Andere als diese Diagnosen wurden in einer fünften Diagnosegruppe (¿andere Diagnosen¿) subsumiert. Für die einzelnen Krankheitsbilder ließen sich unterschiedliche, Vor-und Begeiterkrankungsprofile erstellen. Dabei konnten auch geschlechterspezifische Unterschiede herausgearbeitet werden.\r\n\r\nSchlussfolgerung: Die hier dargestellten Ergebnisse hinsichtlich des Vor- und Begleiterkrankungsprofils können potenziell Grundlagen zu Aussagen über Zusammenhänge zwischen essenziellen Vorbefunden und dem postinterventionellen Outcome bei anästhesiologisch begleiteten neuroradiologischen Interventionen liefern. Um das Stressniveau messen und darstellen zu können, eignet sich die Herzratenvariabilität mit ihrer Vielzahl an Kennwerten. Diese Diplomarbeit befasst sich mit der Fragestellung der kardiovaskulären Reaktionsdynamik einer gesunden Proband*innengruppe, im Rahmen einer Stresssituation. Die Testungen dieser Studie wurden an zwei unterschiedlichen Messtagen durchgeführt, wobei einer der beiden Tage in einer ruhigen Woche ohne Stressor stattfand, um Vergleichswerte in einem relativ entspannten Umfeld zu ermitteln. Der zweite Messtag war jener, an dem es durch einen bevorstehenden „Bungy-Sprung“ möglich war, auch Messungen in der Antizipationsphase, dem Sprung selbst sowie danach zu generieren. Die Daten, welche im Rahmen der Studie erhoben wurden, waren die Herzfrequenz als kardiovaskuläre Größe sowie eine Auswahl der Kennwerte der Herzratenvariabilität (HRV), welche durch die kontinuierliche Messung mittels EKG erhoben wurden. Ein besonderer Aspekt dieser Diplomarbeit war auch die Messung der HF sowie der ausgewählten Kennwerte der HRV während des sogenannten Mittenecker Zeigeversuchs (MPT), welcher als Test für kognitive bzw. exekutive Fähigkeiten ebenfalls im Rahmen der Studie an beiden Messtagen durchgeführt wurde. Zusätzlich widmet sich diese Diplomarbeit auch der Interpretation der Herzfrequenzdynamik während des Bungy-Sprunges und inwiefern es einen Zusammenhang mit dem Aktivierungsniveau der Proband*innen aus dem vorher durchgeführten Mittenecker Pointing Test gibt. Untersucht wurden auch die verschiedenen Aktivierungsniveaus der Proband*innen, sowie die statistisch signifikanten Unterschiede der ausgewählten Kennwerte der HRV sowie der mittleren Herzfrequenz während des MPTs am Bungy-Sprung Tag im Vergleich zum Ruhetag. Trotz des MPTs konnten die Fähnriche auch am Tag des Stressors adäquate Entscheidungen treffen. Die Übersicht über die vier Ruhephasen R0, R1, R2 und R4 an Tag A zeigte ein weitaus höheres Aktivierungsniveau in den beiden Ruhephasen vor dem Bungy-Sprung (R0 und R1) im Vergleich zu den Post-Stressor-Phasen (R2 und R4). Der Vergleich der Ruhephasen nach dem MPT, an beiden Testtagen, deckte die physischen Auswirkungen des drohenden Bungy-Sprunges im Sinne einer Antizipationsphase auf. Die letzte Analyse der Diplomarbeit galt der Herzfrequenzdynamik des Bungy-Sprunges, welche sich graphisch im Sinne der Erholung eindrucksvoll darstellte. Die Ergebnisse dieser Studie zeigten, dass die Proband*innen verschiedene Aktivierungsniveaus aufweisen, jedoch in vielen Phasen der Messungen ähnlich auf Stress reagierten. Wie erwartet, zeigte die Antizipationsphase des Stressors an Tag A bereits bei allen Proband*innen ein erhöhtes Stressniveau mit veränderten Kennwerten der HRV im Vergleich zu Tag B. Anhand der Werte der vier Ruhephasen an Tag A zeigte sich eine Gruppe, welche mit höherem Aktvierungsniveau startete und sich auch gut erholte.
