Nierentransplantationen im Kindesalter stellen eine besondere Herausforderung dar, da die Kinder in den meisten Fällen eine Erwachsenen-Spenderniere erhalten und die gesamte Kreislaufsituation des Kindes an das oft zu große Spenderorgan angepasst werden muss. Daher ist ein optimales intraoperatives Management wichtig, um nach Öffnen der Anastomosen eine primäre Transplantatfunktion erzielen zu können und somit ein weiterer Dialysebedarf verhindert werden kann.\r\n\r\nMethoden: \r\nIn dieser retrospektiven Datenerhebung wurden 47 Kinder und Jugendliche im Alter von 1-19 Jahren eingeschlossen, die im Zeitraum 29.03.2005-01.03.2022 am Universitätsklinikum Graz ein Nierentransplantat erhalten haben. Da es keine evidenzbasierten Empfehlungen des perioperativen Managements bei Nierentransplantationen im Kindesalter gibt, wurden in dieser retrospektiven Studie Daten der perioperativen Kreislaufsituation und des Volumsmanagements in Bezug zum postoperativen Ausgang gestellt. Eine Primärfunktion wurde dadurch definiert, dass postoperativ keine Dialyse notwendig war.\r\n\r\nErgebnisse: \r\nBei 47 Nierentransplantationen zeigten 42 (89%) eine Primärfunktion und fünf (11%) eine Nicht-Primärfunktion. Von den Patient*innen mit Nicht-Primärfunktion hatten drei eine verzögerte, zwei eine fehlende Transplantatfunktion. Die Ursachen für die Nicht-Primärfunktion waren unterschiedlich: Transplantatvenenthrombose, Tacrolimus-Toxizität, prolongierte kalte Ischämiezeit, postoperative Blutung, bzw. insuffiziente perioperative Volumsexpansion. Die Volumsexpansion bis zur Transplantatperfusion lag bei Primärfunktion bei 38,61(±17,81)mL/kgKG. Von dieser Gruppe erhielten Patient*innen mit Over Sized Kidney eine Volumsexpansion von 44,4(±14,44)mL/kgKG, Patient*innen mit Size-matched kidney eine Volumsexpansion von 33(±19,35)mL/kgKG. Bei einer perioperativen Gesamtflüssigkeitszufuhr von über 90mL/kgKG stieg das Risiko für eine Volumenüberladung mit protrahiertem Sauerstoffbedarf auf 28%. \r\n\r\nSchlussfolgerung: \r\nDie ausgewerteten Daten zeigen, dass eine zu niedrige Volumsexpansion zu einer Nicht-Primärfunktion führen kann. Survival depends on the timing, and quality of cardiopulmonary resuscitation (CPR) measures. Some factors associated with a favorable outcome have been defined, but regional differences do not allow a generalization. No cardiac arrest registry has been established in Austria so far. The aim of this retrospective analysis is to provide a systematic overview of OHCA events in the Austrian State of Carinthia in a contemporary setting. \r\n\r\nMaterial and Methods: A retrospective analysis of all medical emergencies that were classified as cardiac arrest, that occurred in Carinthia in 2018, was performed. Original protocols of air, and ground rescue operations performed by the Air Rescue Austria and the Austrian Red Cross Carinthia were evaluated. The clinical course of all OHCA patients that were transported to the hospital was followed by a retrospective analysis of the regional clinical information system.\r\n\r\nResults: A total of 441 OHCA cases were identified, of which 106 (24 %) resulted in return of spontaneous circulation (ROSC), 292 (66 %) in death preceded by CPR, and in 43 (10 %) of the patients, transport with ongoing CPR was initiated. Clinical data of 125 patients with ROSC or that arrived with ongoing CPR were accessible. Of these, 30 patients (24 %) survived with a favorable neurological outcome, two patients (2 %) had a persistent vegetative state, and 93 patients (74 %) did not survive the hospital stay. The median age of all analyzed patients was 75 years (interquartile range [IQR] 62 - 85) and 35 % were female. The median arrival time of the advanced life support (ALS) team was 10 minutes (IQR 7 - 16 minutes). In 47 % of all OHCA cases, bystander CPR was performed. Pre-hospital defibrillation (p= 0.002), younger age (p= 0.032), and shorter duration of CPR (p< 0.001) were associated with increased ROSC rates, while younger age (p= 0.012), shorter duration of CPR (p< 0.001), lower initial blood levels of lactate (p< 0.001), higher blood-pH levels (p< 0.001), and lower serum levels of the neuron-specific enolase (NSE; p= 0.031) were associated with in-hospital survival. In logistic regression, shorter duration of CPR was the strongest predictor for both, preclinical ROSC (p< 0.001) and overall survival (p= 0.033). Among blood biomarkers, lower lactate levels were a predictor of survival (p= 0.012). None of the patients transported with ongoing CPR survived.\r\n\r\nConclusion: The overall survival rate observed in this study was 7.3 %. The ROSC rate of OHCA patients in Carinthia was approximately 24 %. Prehospital defibrillation, younger age, and shorter duration of CPR contributed to a primarily positive outcome of OHCA, whereas age, duration of CPR, and blood levels of lactate, pH, and NSE levels were associated with survival. Schädigung und nicht vollständige Funktionsfähigkeit der Endothelzellen sind als krankheitsauslösenden Ereignisse bei PH anerkannt, jedoch welche spezifischen Faktoren diesen Prozess vorantreiben ist noch nicht vollständig geklärt. Jüngste Erkenntnisse deuten darauf hin, dass eine vermehrte Ablagerung von Komponenten der Basalmembran (BM), einer Untergruppe von Proteinen der extrazellulären Matrix (ECM) unterhalb der pulmonal-arteriellen Endothelzellen (PAEC), bei der Pathophysiologie in PH entscheidend ist. Darüber hinaus kann eine vermehrte BM-Ablagerung auch zu vermehrten Abspaltung BM-Fragmenten führen. Diese biologisch aktiven BM und ECM-Fragmente, genannt Matrikine, haben jedoch eine andere Funktion als die Ursprungsproteine, von welchen sie abgespalten werden. So wurde festgestellt, dass Kollagen IVα5, ein BM-Kollagen, bei PAH-Patienten erhöht ist und die Funktion der PAEC verändert. Die Rolle seines Matrikins, Pentastatin, auf die Funktion der PAEC ist jedoch noch nicht ausreichend geklärt. Ziel dieser Studie war es daher, die Rolle von Pentastatin bei der PAEC-Funktion und seine Bedeutung bei PH zu untersuchen.\r\nBei PH-Patienten im Endstadium korrelierte die erhöhte Genexpression von COL4A5 positiv mit dem mittleren pulmonal-arteriellen Druck, was auf die Schwere der Erkrankung hinweist. Dieser Befund spiegelte sich in der Ablagerung von Kollagen IVα5 in voller Länge und dessen NC1-Fragmenten in umgebauten Lungenarterien wider. Die Behandlung mit synthetischem Pentastatin induzierte eine PAEC-Aktivierung, die durch ein erhöhtes proinflammatorisches Transkriptionsprofil, Apoptose, einen Verlust der endothelialen Barriere Integrität sowie Veränderungen der Adhäsion und Transmigration von Immunzellen gekennzeichnet war.\r\nMechanistisch gesehen setzte Pentastatin β1-Integrin-Rezeptoren in Gang, was zu Integrin-Aktivierung und Clustering führte. Die pharmakologische Blockade von β1-Integrin oder Rho/ROCK schwächte die Pentastatin-induzierte Störung der Endothelbarriere teilweise ab. Die Ätiologie hinter den \r\nBeschwerden weist ein breites Spektrum auf und zeigt eine Abhängigkeit vom Alter der jungen PatientInnen. Eine rasche Einordnung der Beschwerden kann sich als schwierig erweisen, da diagnostische Schritte Zeit in Anspruch nehmen \r\nund zahlreiche pädiatrisch-psychosomatische und psychiatrisch-psychosomatische Krankheitsbilder in Erwägung gezogen werden müssen. Ziel dieser Diplomarbeit ist eine Erstellung eines diagnostischen Konzeptschemas einer \r\ninterdisziplinären abdominellen Schmerzabklärung im Kindes- und Jugendalter.\r\n\r\nMethoden: In dieser Arbeit wird eine Literaturrecherche zu den häufigsten internistischen und psychiatrischen Ursachen und Risikofaktoren abdomineller Schmerzen durchgeführt. Basierend auf den gefundenen Resultaten wird eine \r\nnarrative Konklusion in Form eines interdisziplinären Diagnostikkonzepts abdomineller Schmerzen erstellt.\r\n\r\nErgebnisse: Nach Ausschluss von akut zu versorgenden Krankheiten (Beachtung von Red Flags) kann ein interdisziplinäres Frageschema zur Ermittlung der \r\nEinteilung der vorliegenden Ätiologie angewendet werden. Das zusammengefügte Diagnostikschema beinhaltet die Hauptpunkte Abklärung von Red Flags, Schmerzsymptomatik, Personenangaben, Stuhlverhalten, Begleitsymptomatik, Vorerkrankungen, Familienanamnese, Prädispositions-, Auslöse- und Erhaltungsfaktoren, Laboruntersuchungen und apparative Untersuchungen. Durch Anwendung dieses Schemas können weitere Vorgehensweisen in Form von folgenden internen Abklärungen oder Überweisungen geplant werden.
