Wenig bekannt ist dabei, dass die Suizidraten sowohl in Österreich als auch allen anderen europäischen Ländern stark mit dem Alter ansteigen. Die allerhöchsten Suizidraten finden sich bei hochaltrigen Männern. Österreich hat gegenwärtig eine der höchsten Suizidraten bei Hochaltrigen in ganz Europa und es wird erwartet, dass die absolute Zahl der Suizide bei älteren Menschen in den nächsten Dekaden aufgrund der fortschreitenden Überalterung der Bevölkerung, insbesondere durch das Älterwerden der Babyboomer-Generation, steigen wird. Die zentrale Forschungsfrage dieses Projekts ist daher, welche Risikofaktoren Suizide bei älteren Menschen in Österreich vorhersagen können, was in weiterer Folge dazu beitragen soll, Präventionsmaßnahmen zu verbessern. Vor dem Hintergrund erstmalig zugänglicher und auf individueller Ebene verknüpfbarer Registerdaten in Österreich durch das Austria Micro Data Center (AMDC) soll im Rahmen dieses Projekts untersucht werden, welche älteren Personengruppen ein erhöhtes Risiko für Suizid haben. Konkret planen wir Risikofaktoren in zwei Bevölkerungsgruppen zu untersuchen: (1) die gesamte ältere Allgemeinbevölkerung (Niedrigrisiko-Ansatz) und (2) ältere gerontopsychiatrische Patient*innen (Hochrisiko-Ansatz). Potentielle Risikofaktoren werden demographische und sozioökonomische Faktoren, körperliche und psychische Erkrankungen, Pflegebedürftigkeit und soziale Isolation umfassen. Dabei werden diese Risikofaktoren nicht nur isoliert voneinander betrachtet, sondern auch deren multiplikativer Effekt, d.h. wenn beispielsweise der Verlust eines Partners mit einer neuen Krebsdiagnose zusammenfällt, untersucht. Mit diesem Forschungsprojekt wollen wir den Alterssuizid in Österreich besser verstehen und damit eine bessere Grundlage für Präventionsmaßnahmen schaffen, und nicht zuletzt die Lebensumstände jener älteren Personen besser zu beleuchten, die keinen anderen Ausweg sehen, als sich selbst (oft mit Gewalt) das Leben zu nehmen. ","en":"Suicide is a tragedy at the individual level and a serious public health problem at the societal level. Although often overlooked, suicide rates in both Austria and other European countries increase rapidly with age, so that the highest suicide rates tend to show among the oldest old, and here, specifically among men. Austria has one of the highest suicide rates among the oldest old within the EU-27 countries. The high number of suicide deaths among older adults in Austria is not only a present-day concern, but also worrying against the background of population ageing, specifically the ageing of the large baby boomer generation, as this will likely lead to an increase in the absolute number of suicides among older adults in the decades ahead. The main research question of this project therefore is what risk factors can predict old age suicides in Austria, which can subsequently inform suicide prevention efforts. Against the background of newly available individual-level register data linkage in Austria within the Austrian Micro Data Center (AMDC), the aim of this project is to provide new and hitherto unavailable evidence on who among the older population is at an elevated risk of suicide death. Specifically, we seek to identify risk factors in two populations: (1) the whole general older population (low-risk setting) and (2) older psychiatric inpatients (high-risk setting). Risk factors will include demographic and socioeconomic factors, physical and mental health problems, disability and social disconnectedness. We will not only look at these factors in isolation, but also assess whether there are multiplicative effects for suicide risk, that is, when multiple risk factors co-occur at the same time, e.g. when the loss of spouse coincides with a cancer diagnosis. Checkpoint-Blockade-Therapien (CBTs) haben sich als vielversprechender Weg erwiesen, um der Umgehung der Anti-Tumor-Immunantwort durch den Tumor entgegenzuwirken. Trotz ihres Potenzials sind CBTs mit erheblichen Kosten verbunden und basieren häufig auf Trial-and-Error-Ansätzen. Zudem sprechen nur ein kleiner Prozentsatz der Lymphom-Patienten (10-20%) auf CBT an, teilweise aufgrund eines Mangels an Verständnis darüber, wie Immun-Checkpoints während der Entwicklung von Lymphomen reguliert werden, sowie aufgrund des Fehlens prädiktiver Biomarker. Unsere jüngsten Forschungsergebnisse haben wichtige Faktoren aufgedeckt, die zu schlechten Ergebnissen bei Patienten mit diffusen großzelligen B-Zell-Lymphomen (DLBCL) beitragen. Wir beobachteten signifikante Abnahmen der NR4A1- und p53-Spiegel, die wichtige Regulatoren der Genexpression sind, vergesellschaftet mit erhöhten Checkpoint-Komponenten-Gehalt. Bei Mäusen führte die Deletion von NR4A1 zu reduzierten p53-Spiegeln, beschleunigter Lymphomentwicklung und erhöhter Expression von immuninhibitorischen Checkpoint-Komponenten. Diese Ergebnisse deuten auf eine funktionelle Beziehung zwischen NR4A1 und p53, die in der Immunantwort auf Tumore die Expression von Checkpoint-Komponenten in aggressiven Lymphomen reguliert.\r\nUnser vorgeschlagenes Forschungsprojekt zielt darauf ab, diese Herausforderungen durch die Entwicklung neuer Mausmodelle mit spezifischen Deletionen von NR4A1 und/oder p53 in B-Zellen anzugehen. Unter Verwendung fortschrittlicher molekularer und genomweiter Techniken werden wir umfassend untersuchen, wie Lymphome in diesen Modellen entstehen. Außerdem werden wir eine Methode zur wiederholten Gewebsflüssigkeitsentnahme adaptieren und damit wiederholte Messungen von Zytokinsignalen und zellulären Veränderungen in sich entwickelnden Tumoren durchführen, wodurch wertvolle Einblicke in das Lymphom-Mikromilieu gewonnen werden.  Mit Hilfe dieser Modelle hoffen wir, neue Checkpoint-Blockade-Therapien für aggressive Lymphom entwickeln und auf den Status von NR4A1 und p53 abstimmen zu können. Zur Validierung unserer Ergebnisse werden wir eine große Kohorte von Lymphomproben von Patienten analysieren und mit öffentlich verfügbaren Datensätzen vergleichen. Letztendlich ist es unser Ziel, die klinische Relevanz der NR4A1-p53-Checkpoint-Achse zu bewerten und neue Biomarker zu identifizieren, die bei aggressiven Lymphomen als Entscheidungshilfe für die Behandlung mit Checkpoint-Inhibitoren dienen können. Durch unsere Forschung wollen wir die Ergebnisse für Patienten mit aggressiven Lymphomen verbessern, indem wir wirksamere und personalisierte Behandlungsstrategien entwickeln.\r\n","en":"Aggressive lymphomas are among the most common types of lymphoid cancers and consist of Burkitt’s lymphoma, diffuse large B cell lymphoma (DLBCL), follicular lymphoma grade 3. While checkpoint blockade therapies (CBTs) have emerged as a promising avenue to counteract tumor evasion, this treatment form comes with substantial cost burdens and often rely on trial-and-error approaches. Importantly, only a small percentage of lymphoma patients (10-20%) respond favorably to CBT, partly due to the limited understanding of the regulation of immune checkpoints during lymphoma development and the absence of predictive biomarkers. Our recent investigations have shed light on key factors contributing to poor outcomes in patients with DLBCL. We observed significant decreases in the levels of NR4A1 and p53, which are important regulators of gene expression and known tumor suppressors, alongside high expression of checkpoint components. In mice, depletion of NR4A1 led to reduced p53 levels, accelerated lymphoma development, and increased expression of immune-inhibitory checkpoint components, particularly in animals with intact immune systems. These and other findings suggest a functional relationship involving NR4A1, p53, and the orchestrating immune evasive mechanisms in aggressive lymphomas.\r\nOur proposed research project aims to address this axis by developing new mouse models with specific and inducible deletion of NR4A1 and/or p53 in B cells. Using advanced molecular and genomic techniques, we will comprehensively study lymphoma development in these models. We will also implement a tissue fluid sampling method to repeatedly measure cytokine signals and cellular changes within evolving tumors, providing valuable insights into the lymphoma microenvironment. By leveraging these models, we aim to tailor CBTs based on the status of NR4A1 and p53. To clinically validate our findings, we will analyze a large cohort of lymphoma samples from GRAZ lymphoma cohort and compare them with publicly available datasets. Ultimately, our goal is to functionally establish the clinical relevance of the NR4A1-p53-checkpoint axis and identify new biomarkers that can help guide treatment decisions involving checkpoint inhibitors for aggressive lymphomas. Through our research, we are poised to improve outcomes for patients with aggressive lymphomas by developing more effective and personalized treatment
